32 | LPA observed fully stocked first aid kit. LPA Lipsey observed a working carbon/smoke detector in the hallway and living room, LPA was able to hear a successful test. Fire extinguisher 2A10BC was observed and located in the hall cabinet and was purchased 3/3/25. The home was observed to be clean and orderly with heating and ventilation. LPA observed safe toys, play equipment, and materials. LPA verified the home has a working cell service.
LPA reviewed a sample of the children’s records. For (LIC) 282- Affidavit Regarding Liability Insurance, which was not present, Immunizations Records, LIC 700- Identification and Emergency Information, LIC 627- Consent for Medical Treatment, LIC 995A Notification of Parents’ Rights, which were missing parents’ signatures and children’s names. The LIC 9227- Infant sleep form (0-12 months, and documentation of 15-minute Infant Sleep Check (0-24 months) was not completed due to licensee does not presently have any infants.
Staff files were in order. Staff records were reviewed for approved Pediatric First Aid and CPR certification, LIC-501: Personnel Record, LIC 508-Criminal Record Statement, LIC 9052- Employee Rights, Proof of immunizations against measles, pertussis and influenza or influenza declination, TB clearance or risk assessment, LIC 9108- Statement Acknowledging Requirement to Report Child Abuse and current Mandated Reporter Training Certificate. Licensee’s pediatric CPR/First Aid was completed on 7/25/23 and Mandated Reporter Training was on Completed on 9/25/23.
On this date, 10/13/23 the California Attorney General - Megan's Law website was searched for information on sex offenders required to register with local law enforcement under California's Megan's Law. No registered sex offenders were found at the facility addresses. Under state law, some registered sex offenders are not subject to public disclosure; therefore, they may not have been included in this search. However, the Department conducts a monthly cross reference of each address on record for all registered sex offenders against all CCLD facility addresses pursuant to information shared by California DOJ.
