Annual Random Visit Conducted In Armenian

Licensing Program Analyst (LPA) Silva Garibyan conducted a site visit for the purpose of an Annual Random visit. The licensee has applied for capacity increase. Applicant has proof of teaching experience and qualifies for a large family child care license. LPA met with the licensee and toured the home inside and outside at 12:00 p.m. on 09/20/2024. The licensee was present with five children ( including two infants) and one assistant. The licensee has installed the pull fire alarm that is required for large Family Child Care Homes. Applicant has an approved fire clearance. There are no changes to the facility layout or to the off limit areas. Per applicant operation hours are Monday to Friday from 7:30 am to 6:00 pm. Licensee cares for children newborn to 13 years of age. Entrance Checklist was provided to the Licensee. Per Licensee a cell phone with active service in the home is the main contact number while children are in care. All areas identified on the facility sketch were inspected, including but not limited to, off limit areas. This is a single story home that consists of 3 bedrooms, 2 bathrooms, living room, dining room, kitchen, laundry room, attached garage. There is a pool in the back yard. LPA took photos of the pool fencing. Measurements were taken and documented to ensure that the fencing meets Title 22 requirements. The pool mesh fencing measured 5 feet high; has a self-closing, self latching gate that swings away from the pool and has a self latching device that is 3 inches from the top of the fence. The mesh fence encloses the pool on 2 sides. Two sides are enclosed by the property wall. The spacing between railings is no more than 4 inches. The distance between the ground and the bottom of the fence is no more than 4 inches from the hard surface (concrete). The mesh fencing is sturdy and capable of withstanding the impact of children's toys including bicycles. The mesh fence does not readily bend upon impact. Applicant agrees that the fencing will remain in place whenever licensed care is provided, and so long as the mesh fence makes the swimming pool inaccessible to children. The gate latch has a key lock and applicant has stated that the gate is normally locked and that she carries the key on her person. There are no other bodies of water on the premises.

Per Licensee, the children will use the following areas: Living room, Dining room, and bathroom (1) in the hall way. Licensee has designated Bedroom(1), Bedroom(2), Bedroom (3), Bathroom(2), Laundry room, and Garage as OFF LIMIT to the children in care. LPA observed bedroom doors with safety knob/locks making off limit areas inaccessible. The bathroom that children use is in the hall way. A toilet, a sink and a bath tub was observed. The bathroom was observed to be free of hazards. The home is equipped with a central heating/cooling system. Sleeping area for children was observed in the living room. Living room has a fireplace which has been screened to prevent access by children. Per Licensee, the fireplace will not be used. All areas identified on the facility sketch as accessible to children were inspected to ensure that they are clean and orderly with ventilation and heating for the safety of the children. LPA observed age appropriate toys, free of loose and sharp parts. LPA observed electrical covered outlets all through the home. Detergents, cleaning compounds, medications, and other items which can pose a danger to children are inaccessible. Per licensee, there are no pets and LPA did not observe any pets at the time of visit. Per Licensee, isolation area for sick children is in the dining room. Licensee has designated the back yard as the outdoor play area for children. LPA observed artificial grass and adequate perimeter fencing through-out the yard. LPA advised Licensee that outdoor supervision is required at all times. Each family child care home shall conduct fire drills and disaster drills at least once every six months. Disaster drill log was available during today’s inspection, last disaster drill conducted on 08/13/24. Per licensee, the facility provides food for children in care (breakfast, lunch, and PM snack). Food preparation areas were toured for safety, cleanliness and proper equipment. LPA discuss food preparation, storage and ensuring a log and information regarding dietary restrictions and allergies are kept up to date.
The valve on the required 2A 10BC fire extinguisher indicates fully charged. The smoke and carbon monoxide detector (two in one) was tested and operable at the time of visit. Per Licensee, the First Aid kit is kept in the upper kitchen cabinet, making it inaccessible to children. The First Aid kit was inventoried for necessary supplies. LPA observed tables, chairs and napping equipment. Licensee reports she has no firearms or weapons in the home. LPA also observed Licensee's and Assistant's current Pediatric CPR (Adult/Infant /Child) and Pediatric First Aid certifications (expire 10/2025) and Mandated Reporter Training (completed on 10/16/2023 and 07/19/2024). LPA observed in the living room the Parent Board with all necessary posting required ( Facility License (LIC 203), Emergency Disaster Plan (LIC610A), Notification of Parents' Rights Poster (PUB 394), If You see Something Say Something poster, Car Seat Safety poster). Child Care Facility Roster (LIC9040) was on file.

A review of the children's records was conducted and are found to have the following: LIC 282 Affidavit Liability Insurance, LIC 627/Consent for Medical Treatment, LIC 700/ID and Emergency Information, LIC702, LIC 995A/Parent's Rights, LIC995E/Caregiver Background Check, LIC 9150/Parent Notification, LIC 9212/Parent's Responsibilities, PM 286/Immunization Card. Sleep logs for infants on file.

A review of records indicates that all employees and/or volunteers have immunization records on file for influenza, pertussis and measles. All adults who reside or work in the home have a criminal record clearance or exemption. There are no excluded individuals present at this home.

Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

Two infants are enrolled at this time. LPA discussed the safe sleep regulations with assistant and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource.
LPA also informed assistant of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

During the exit interview, the assistant Armine Azaryan confirmed that there are no Registered Sex Offenders living in the facility.
A notice of site visit was given and must remain posted for 30 days.
