Licensing Program Analyst (LPA) Tatiana Bickham conducted an announced pre-licensing inspection for a small family childcare at the above facility on 05/08/2024 at 8:10 AM. LPA met with Applicants Davit Davtyan and Hripsime Melikyan, who guided analyst on a tour of the facility. There were no children present when LPA arrived. Per Applicant, hours of operation will be Monday through Saturday, 7:00am to 10:00pm. Applicant states they will provide care for children 3 months to 13 years of age.

This is a single home consisting of 3 bedrooms, 1 1/2 bathrooms, living room, dining room, kitchen, family room (day-care room #1) detached room (day-care room #2), laundry room, front yard (driveway), and backyard. The home was inspected for safety, cleanliness, telephone service, heating and ventilation, inaccessibility to poisons, detergents, cleaning compounds, medicines, and hazardous items that can pose a danger to children.

Areas used by children include: Family room (day-care room #1) accessible through the dining room, bathroom located in the hallway across from bedrooms, detached room (day care #2) adjacent from day-care room #1 and backyard, half of the backyard will be used and there will be a gate separating the off limits portion of the backyard. Areas off limits include: All 3 bedrooms, ½ bathroom located in master bedroom, dining area, living room, portion of the backyard, laundry room and kitchen. The laundry room is located in day-care room #2 and is made inaccessible by locked door. Off limit areas are made inaccessible by door lock or gate. Knives and other sharp utensils are in the kitchen cabinets and drawers, kitchen is made inaccessible by gate. Cleaning products are made inaccessible by being stored in the off limits areas of the kitchen and the laundry room. Parents will use the side gate (locked) located to the right of the home that leads to the backyard and day-care room #1 and #2 to drop off and pick up children.

The home is clean and orderly, there are outlet covers throughout the home. Smoke/carbon monoxide detector was observed, tested and found to be operable.

Applicants have completed the required health and safety training, the Pediatric First Aid and CPR. Applicants have proof of immunization against pertussis and measles, and influenza on file. Proof of Mandated Reporter Training was also provided.

Applicant states they will provide food for the children. LPA reminded Applicant that food brought from home should be labeled with the child's name and properly refrigerated. LPA observed age-appropriate toys and learning materials. The required (2A10BC) fire extinguisher was observed in day-care room #1 and #2 and was purchased in May 2024. Per Applicant, there are no weapons or firearms in the facility.



Per Applicant, the children will use the backyard for outdoor play. The outdoor play area was observed to have a fence around the parameter. The outdoor area is currently missing the fence that will separate the off limits portion of the backyard. There are no pools or spas, or other bodies of water.

The isolation area for a sick children waiting to be picked up will be in the day-care room #1 or #2, away from other children. LPA reviewed with applicant the LIC 311D, Forms/Records To Keep In Your Family Child Care Homes, children’s forms/records, facility forms/records, and information to be posted.



The Applicant provided proof of control of property. Because the applicant leases the home, proof of landlord notification is required. The LPA observed the Property Owner/Landlord Notification form (LIC9151) that the applicant confirms was provided to the property owner. The applicant obtained a signed Property Landlord Consent form (LIC 9149).

Licensee reminded Applicants that all adults 18 and over living in the home, persons who provide care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.

LPA informed Applicants any unusual incidents or injuries must be reported to the Department within 24 hours via telephone and within seven (7) days in writing. Fire and safety drills must be performed every six months and documented for review by the Department and smoking tobacco is not allowed in a home that is licensed as a family day care home, and in those areas of the family day care home where children are

A current roster of children enrolled must be available and maintained for a period of three years, even after children no longer are attending the facility.
Annual fees must be paid promptly and by the due date or a late fee shall be assessed, and/or the License shall be terminated.
The fire extinguisher type 2A-10BC must be serviced annually or as often as necessary. Smoke and carbon monoxide detectors should be checked, batteries should be replaced.
Changes should be reported the to the Department as soon as they occur such as construction and remodeling.

Applicant(s) was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain childcare by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.


LPA discussed the safe sleep regulations with applicant and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources /safe-sleep as an additional resource. LPA also informed applicant of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

On this date, 2/15/24, the California Attorney General - Megan’s Law website was searched for information on sex offenders required to register with local law enforcement under California's Megan's Law. No registered sex offenders were found at the facility addresses. Under state law, some registered sex offenders are not subject to public disclosure; therefore, they may not have been included in this search. However, the

Department conducts a monthly cross reference of each address on record for all registered sex offenders against all CCLD facility addresses pursuant to information shared by California DOJ.

A notice of site visit was given to Licensee and must remain posted on, or immediately adjacent to, the interior side of the main door for 30 days.

Prior to Licensure the following is required, and the applicant will complete by 5/24/2024.
1.Backyard needs fence separting the off limits area.
2. Plastic pipe in the grass on the on limits part of the backyard needs to be fenced or removed.
3. Crib or play yard needs to be purchased for infants.

A small family childcare license may be granted upon receipt of proof of corrections for the above and manager approval. Once licensed, the applicant is required to adhere to the terms and limitations stated on the license.
