On 04/06/2026 at 9:00 a.m., Licensing Program Analysts (LPAs) DeCondia Ferguson and Tatiana Bickham conducted an unannounced annual/random inspection at Davtyan & Melikyan Family Child Care (Kids World Day Care). LPAs met with licensees, Davit Davtyan and Hripsime Melikyan, who provided a tour of the facility.

During the inspection, LPAs observed child care being provided in two rooms. A total of three children were present and supervised by two staff. The licensees stated that there are nine children enrolled. Two children are part-time and leave at 2:30 p.m., while six additional children arrive at 2:30 p.m. The facility operates Monday through Saturday from 7:30 a.m. to 10:00 p.m.

A walkthrough of the facility was conducted. The three bedrooms, ensuite bathroom, kitchen, and laundry room are off-limits. Areas on-limits include the living room, dining room, two daycare activity rooms, and a portion of the backyard. There are no pools or bodies of water on the premises.

The activity areas were observed to be clean and free of hazards. LPAs informed the licensees that daycare room #1 (detached converted garage) shall not be used for napping, as it cannot be accessed through the home. Napping will take place in daycare room #2. Furniture was observed to be in good repair and age-appropriate. The facility has adequate heating, lighting, and ventilation.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

A properly supplied first aid kit was observed in daycare room #2. The bathroom and toileting areas were inspected and found to be clean, with fixtures accessible to children. Adequate lighting and ventilation were also observed.

The facility provides breakfast, lunch, dinner, and two snacks daily. The food preparation area was inspected for safety, cleanliness, and proper equipment.

The facility has at least one functioning fire extinguisher and smoke/carbon monoxide detectors that meet statutory requirements. However, the fire extinguishers require servicing, as no current service record was available. Smoke and carbon monoxide detectors were tested and found to be operational. The facility was operating within its licensed capacity and limitations. At least one individual present was certified in CPR and Pediatric First Aid.

Children’s and staff files were reviewed and contained required documentation. Disaster drill requirements and documentation were discussed. Mandated reporter responsibilities were reviewed. The mandated reporter training on file expires on 04/21/2026; licensees were advised to renew prior to expiration.

The licensees were reminded of their responsibility to understand and comply with all applicable regulations, including ensuring that all assisting individuals are also knowledgeable of requirements. Information on accessing regulations, forms, and updates was provided via www.ccld.ca.gov.

The licensees were advised that accessibility of hazards must be continually reassessed based on the children in care. Licensing must be notified of any changes to the facility phone number. Smoking is prohibited on the premises; licensees stated there is no smoking in the home. Licensees also stated there are no firearms on the premises.

Criminal Record Clearance
Family Child Care Homes Licensee’s was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

Assembly Bill 633: Upon receipt of a citation for a Type A Deficiency, the director shall post the licensing report for 30 days in addition to the Notice of Site Visit & provide copies of the licensing report (LIC 809D/9099D) to parents/guardians of children in care at the facility by the close of business the following day or the next day child returns to the facility. The same report must be provided to parents/guardians of children newly enrolled at the facility during the next 12 months & licensee will obtain a signed Acknowledgement of Licensing Reports (LIC 9224) from parent/guardian & place it in each child's file.

Senate Bill 792: This bill, commencing September 1, 2016, prohibits a person from being employed or volunteering at a child care facility or family day care if he or she has not been immunized against influenza, pertussis and measles.

New Immunization Requirement: Law enacted by SB 277, beginning January 1, 2016, personal beliefs exemptions will no longer be an option for the vaccines that are currently required for entry into child care or school in California. Personal beliefs exemptions already on file will remain valid until the child reaches the next immunization checkpoint.

New Appeal Process: A licensee may file an appeal, in writing 15 business days from the date of receiving the penalty assessment. All appeals must be sent to:

Assembly Bill 1207: California Child Care Workers; Mandated Training Requirement. Beginning January 1, 2018, all licensed providers, applicants, directors and employees must complete Mandated Reported Training prior to March 30, 2018 and renew training every two years at: www.mandatedreporterca.com.

Nutrition Requirement: Beginning January 1, 2016, AB 290 will require for each new license issued, at least one director or teacher at each child care center or family child care home to have at least one hour of training in the importance of childhood nutrition. This applies to anyone submitting a new application, relocating their facility, selling their facility or transferring their license. Please note this training cannot be completed online or by home study programs. The training must be taken from an Emergency Medical Services Authority (EMSA) approved training program OR an accredited college or university.

Licensee’s was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

During the exit interview, the LICENSEE’S, confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS 04/06/2026.
