On 10/16/2024 Licensing Program Analysts (LPA’s) Laticia Thompson and Lilia Hernandez met with Carmen Vazquez , Director. The purpose of the visit was an announced pre-licensing inspection at the facility to ensure that health, safety and personal rights as required by Title 22 and Health and Safety Regulations governing California Child Care Centers will be met. There were XX children present during this inspection. This is an application for a Single License with a Total capacity of 144; 12 infants ages 0 – 24 months, 12 Toddlers 18 – 36 mos and 120 Preschoolers, ages 2 years into entry of 1st grade. The child care center will operate Monday – Friday 7:00 a.m. – 5:30 p.m. LPA's toured and measured the on-limits areas. The fire clearance was approved on 10/10/2024. The facility has a total of 7 classrooms. Classroom 7 will be dedicated to the infant program, classroom 6 will be dedicated to the Toddlers and classrooms 1-5 will be dedicated to the preschool. Toddlers and Infants have a joining bathroom and will share 2 sinks and 2 toilets. Preschoolers have access to 10 toilets and 10 sinks. Staff restrooms are separated and located in each individual classrooms.

The Indoor total measurements contain a total of 729.62 which will accommodate Applicant’s request for 12 children.

Toddler Indoor measurements are:

The Indoor total measurements contain a total of 843.55 which will accommodate Applicant’s request for 12 Toddlers.

The Indoor total measurements contain a total of 4,561.53 which will accommodate Applicant’s request for 120 Preschool children.

Infant Outdoor Activity measurements are:

The outdoor activity space total measurements contain a total of 2,143.46 which will accommodate Applicant’s request for 12 infants.

Toddler Outdoor Activity measurements are:

The outdoor activity space total measurements contain a total of 2,143.46 which will accommodate Applicant’s request for 12 Toddlers.

Preschool Outdoor Activity measurements are:

The outdoor activity space total measurements contain a total 18, 235.84 of which will accommodate Applicant’s request for 120 Preschool children.

The following was observed:



Parents will enter and exit through the gate located outside of the preschool outdoor play area. The gate has intercom communication to access the premises.

Parents will sign in and out manually in the front office. There is an operable carbon monoxide detector, fire extinguishers with inspection dates of 01/29/2024, first aid kits with the required materials, mats for napping, cubbies for storage of children personal belongings, central air for heating and cooling.



Furniture and equipment were observed to be maintained in good condition free of sharp, loose or pointed parts. Tables and chairs were observed to meet the needs of the children. Classrooms are clean, has adequate lighting, windows are in good repair free of chipping paint, dirt, insects, or debris. Play equipment and materials used by children were observed to be age-appropriate

The facility is equipped with landline telephones service. Disinfectants, cleaning solutions, and other items that are dangerous to children are stored in a large locked metal container located on the play yard area inaccessible to children. Ill children will use the office and office staff restroom for isolation. The isolation area shall be equipped with a cot for ill children and shall be supervised by staff. Medication that requires refrigeration will be stored in the refrigerator in the office inaccessible to children.

The required postings are posted in an area where parents and visitors to review. Facility has a disaster and mass casualty plan of action. LPA's did not observe any bodies of water. Transportation is provided through Los Angeles Unified School District.
The program provides breakfast, lunch snacks. The kitchen and food-preparation storage areas were observed to be clean and free of litter and rubbish. Menus are in writing and are posted in advance in an area accessible for review by the child's authorized representative. Copies of the menus as served shall be dated and kept on file for at least 30 days. Menus shall be made available for review by the child's authorized representative and the Department upon request.

INFANT INFORMATION


The sleeping area for infants are physically separate from the indoor activity space. There are a total of 5 cribs. LPA advised director that Cribs shall not limit the ability of staff to see the infant. There are 2 changing tables that are maintained in good repair and safe condition Changing tables has a padded surface no less than one-inch thick, covered with washable vinyl/plastic has raised sides at least three inches high. There is a refrigerator located in the infant kitchenette. Bottles, dishes and containers of food brought by the infant's authorized representative shall be labeled with the infant's name and the current date.

LPA reviewed and provided applicant will copies and postings of Title 22 regulations related to infant safe sleep, infant needs and service plans, staff ratios and infant sleeping equipment.

LPA advised applicant of the following regarding the Toddler Program:101216.4(a)(1) Preschool Program with Toddler Component A child who is between 18 months and 36 months of age may participate in the toddler program with written permission from the child's authorized representative. No child in the toddler program shall be placed in the preschool program before the age of 30 months without written permission from the child's authorized representative.

Outdoor

The entire outdoor activity grounds are enclosed by a fence to protect children and to keep them in the outdoor activity area. The fence is at least four feet high. The surface of the outdoor activity space has natural grass and rubber platforms for cushioning to prevent injury from falls. The outdoor activity space was observed and there were maintained in good condition. There is adequate shade in the outdoor activity areas. LPA observed benches for resting. Director was advised to inspect sandbox and outside area t daily to ensure areas are free from any hazardous foreign material

The following items were discussed:

The director and applicant was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

LPA reviewed with facility representative the LIC 311A, Records To BDocument Link Icone Maintained At The Facility, for child’s records, personnel records, administrative records, and documents to be posted.

Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test.

For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP).

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of “medication and equipment/supplies, and reviewed children’s, personnel, and
administrative records.
