On 04/24/2025 Licensing Program Analysts (LPA) Laticia Thompson met with Ani Saghatelyan, Applicant. The purpose of the visit was an announced pre-licensing inspection at the facility to ensure that health, safety and personal rights as required by Title 22 and Health and Safety Regulations governing California Child Care Centers will be met. The facility is located on the grounds of Jesus Sacred Heart Church. This is an application for a Single License with a Total capacity of 42 Preschool Children ages 2- 6 years. The child care center will operate Monday – Friday 8:00 a.m. – 6:00 p.m. There were no children present during this inspection. The fire clearance was approved on 10/25/2025. The facility has a total of 3 classrooms. Preschool children will have access to 4 sinks and 4 toilets. LPA toured and measured the on-limits areas.

The Indoor measurements contain a total of 1,549 which will accommodate Applicant’s request for 42 Preschool children.

The outdoor activity space total measurements contain a total of 3,623.66 which will accommodate Applicant’s request for 42 Preschool children

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

The following was observed:

Parents will enter and exit through the doors located next to the church entrance and in the parking lot area on Satsuma ave. There will be a barricade to prevent parents from entering the rooms and building utilized by the church.

Parents will sign children in and out electronically under the Bright Wheel application. LPA advised facility there should be a plan in place on how such records will be immediately printable upon request for inspection and audit by program staff. For best practice purposes, facilities should have a plan in place on how records would be backed up, and how they would be made available in the event of a power failure or disaster. LPA reference director to review PIN 18-03.1-CCP.



The facility has a filtered water dispensers where children can fill their personal water bottles for outdoor and indoor activities. There is a water fountain located on the play yard area. LPA advised facility to ensure lead poisoning regulations are met. There is an operable carbon monoxide detector located in the hallway next to classrooms #1 and #2. LPA observed fire extinguishers with an inspection date of 10/17/2024. LPA observed First Aid Kits with the required materials including a handheld digital thermometer.

Furniture and equipment were observed to be maintained in good condition free of sharp, loose or pointed parts. Tables and chairs were observed to meet the needs of the children. There are a sufficient number of cots for napping observed to be maintained in a safe condition. There is sufficient quantity and variety of age- appropriate play equipment in good condition. There are hooks and cubbies in all classrooms for storage of children clothing and personal belongings. The classrooms have central air for heating and cooling. The classrooms were clean in good repair. Classrooms have adequate lighting. Windows are in need of repair from chipping paint. LPA observed trash cans with tight fitting lids. Applicant stated the facility will be equipped with a cellphone dedicated to the facility and will remain at the facility during operating hours.

Disinfectants, cleaning solutions, and other items that are dangerous to children are stored in the director’s office in a locked storage closet inaccessible to children.

The staff restrooms stalls are located next door to the children’s restrooms. Applicant was reminded that children restrooms are not to be shared with staff. All toilets, handwashing and bathing facilities are maintained in safe and sanitary operating condition. Hand washing fixtures do not deliver hot water.


The applicant shall be responsible for ensuring that children with obvious symptoms of illness are not accepted. The isolation area for Ill children will be located in the director’s office. The isolation area shall be equipped with a cot for ill children and shall be supervised by staff. The staff restroom is designated for children who are ill.
The applicant has developed and implemented a written plan to record the administration of prescription and nonprescription medications, to inform the child's authorized representative daily when such medications have been given. Medication will be stored in the padded lock kitchen cabinet inaccessible to children. Medication that requires refrigeration will be stored in the kitchen refrigerator inaccessible to children.

The program provides breakfast lunch, snacks and supper. The kitchen and food-preparation storage areas were observed to be clean and free of litter and rubbish. All foods or beverages that require storage at a temperature of 45 degrees will be stored in the kitchen refrigerator designated to the preschool. The refrigerator is equipped with a thermostat to regulate temperature control. Menus are in writing and are posted in advance in an area accessible for review by the child's authorized representative. Copies of the menus as served shall be dated and kept on file for at least 30 days. Menus shall be made available for review by the child's authorized representative and the Department upon request.

The required postings are not posted for parents and visitors to review. Facility does not have a disaster and mass casualty plan of action. The plan shall be in writing and shall be readily available with contingency plans for action during fires, floods and earthquakes.

The facility does not provide transportation. Firearms and other weapons shall not be allowed on or stored on the premises of a child care. Applicant stated there aren’t any firearms, weapons or bodies of water on the premises. Smoking is prohibited on the premises of a child care center as specified in Health and Safety Code Section 1596.795(b). Licensees shall reveal each child care center license number in all advertisements in accordance with Health and Safety Code Section 1596.861

Outdoor

The entire outdoor activity grounds are enclosed by a fence to protect children and to keep them in the outdoor activity area. The fence is at least four feet high. Areas around and under climbing equipment, swings, slides and other similar equipment has artificial grass, for cushioning to prevent injury from falls. The outdoor activity space was observed and maintained in good condition LPA did not observe any equipment or hazards in the outdoor activity area. Applicant will ensure children will not have access to the playground platform equipment that does not have any railing. There is adequate shade and benches for resting in the outdoor activity area. LPA did not observe any bodies of water.

The following items were discussed:



The applicant was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test.
