At 8:25AM on 10/3/25, Licensing Program Analyst (LPA) Angela Luz conducted an unannounced complaint investigation to deliver findings on the allegation above. LPA toured the facility by themselves for census. LPA arrived during the drop off period and observed four infants with three staff and 13 preschool aged children with three staff. At 9:20 AM Licensee, Anna Baghramyan, arrived to the facility. LPA explained the purpose of today's visit with them.

Throughout the investigation, LPA obtained the children’s roster, personnel report, and sign in and out sheets. LPA reviewed teacher qualifications and conducted interviews and observations.

Reporting Party (RP) alleged that their child's classroom was over ratio during drop off.

On 9/5/25 LPA observed drop off time. At 8:00AM Staff 1 was alone with 5 infant children. Staff 3, who is usually in the infant room, was preparing breakfast for the whole facility in the kitchen area outside the office. At 8:04AM Staff 5 accepted a 13th preschool aged child into care. At 8:07AM Staff 5 accepted an infant child into care. Staff 5 was alone with 13 preschoolers and 1 infant child. At 8:23AM Staff 1 was alone with 8 infant children. Staff 3 was delivering breakfast to the classrooms.

Based on LPAs observations and interviews which were conducted and record reviews, the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED. California Code of Regulations, Title 22, Division 12 Chapter 1 is being cited on the attached LIC 9099D.

A notice of site visit was given and must remain posted for 30 days. Appeal Rights provided.

At 9:05AM Licensee, Anna Baghramyan, arrived to the facility. LPA explained the purpose of today's visit with them.

Reporting Party (RP) alleged the facility and its floors were dirty.

During the investigation, LPA conducted interviews and made observations. During interviews, LPA learned that staff clean their classrooms throughout the day using disinfecting wipes. The facility has one custodian who is responsible for cleaning the facility after hours or when children are no longer in care. On 9/5/25, LPA observed the cleanliness of floors at drop off and after meals.

Based on observation on 9/5/25, Staff 5 (S5) had 13 preschool children and one infant child. This poses an immediate health, safety, or personal rights risk to persons in care.
