On 06/12/2025 Licensing Program Analysts (LPA) Laticia Thompson met with Alyssa Lao, Director. The purpose of the visit was an announced pre-licensing inspection at the facility to ensure that health, safety and personal rights as required by Title 22 and Health and Safety Regulations governing California Child Care Centers will be met. There were 18 infants and 6 staff members present during this inspection. This is an application for a Single License with a Total capacity of 56 infants ages 6 weeks – 24 months. The child care center will operate Monday – Friday 7:00 a.m. – 6:00 p.m. LPA toured and measured the on-limits areas. The fire clearance was approved on 01/21/2025. The facility has a total of 6 classrooms. Classroom 3 is used as a napping/crib room. There is a bonus room used for activities. Infants will have access to 5 sinks, 4 toilets.

Infant Indoor measurements are:

The Indoor total measurements contain a total of 2,242.20 which will accommodate Applicant’s request for 56 infants.

Infant Outdoor Activity measurements are:

The outdoor activity space total measurements contain a total of 6,410.22 which will accommodate Applicant’s request for 56 infants.

The following was observed:

Parents will enter and exit through the front gate with a privacy code.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:

Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.

Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

The facility has filtered water dispensers where children can fill their personal water bottles for outdoor and indoor activities. There are operable carbon monoxide detectors. LPA observed fire extinguishers with an inspection date of 01/23/2025. LPA observed First Aid Kits without all required materials excluding a thermometer.

Furniture and equipment were observed to be maintained in good condition free of sharp, loose or pointed parts. Tables and chairs were observed to meet the needs of the children. There are cots for napping observed and maintained in a safe condition. There are cubbies and hooks for storage of children clothing and personal belongings. The classrooms have central air for heating and cooling. The classrooms were clean in good repair. Classrooms have adequate lighting. Windows were in good repair free of chipping paint, dirt, insects, or debris. LPA observed trash cans with tight fitting lids. The facility is equipped with landline telephones service. Disinfectants, cleaning solutions, and other items that are dangerous to children are stored in cabinet areas with safety hooks inaccessible to children.

All toilets and handwashing facilities are maintained in safe and sanitary operating condition. Director was advised to adjust water temperature for fixtures that deliver hot water.

The applicant shall be responsible for ensuring that children with obvious symptoms of illness are not accepted. The isolation area for Ill children will be located in the bonus activity room and staff restroom located next to the kitchen area. The isolation area shall be equipped with a cot for ill children and shall be supervised by staff. The applicant was advised to develop and implement a written plan to record the administration of prescription and nonprescription medications, to inform the child's authorized representative daily when such medications have been given. Medication will be stored in the kitchen areas inaccessible to children. Medication that requires refrigeration will be stored in the refrigerators inside a locked box.

The program provides breakfast snacks. The kitchen and food-preparation storage areas were observed to be clean and free of litter and rubbish. All foods or beverages that require storage at a temperature of 45 degrees will be stored in the refrigerators. The refrigerators are equipped with a thermostat to regulate temperature control Menus are not posted in writing and are not posted in advance in an area accessible for review by the child's authorized representative. LPA advised director that copies of the menus as served shall be dated and kept on file for at least 30 days. Menus shall be made available for review by the child's authorized representative and the Department upon request.

The all required postings are not posted in an area for parents and visitors to review. Facility does not have a disaster and mass casualty plan of action. LPA advised Director that the plan shall be in writing and shall be readily available with contingency plans for action during fires, floods and earthquakes.

The facility does not provide transportation. Firearms and other weapons shall not be allowed on or stored on the premises of a child care. The director stated there aren’t any firearms or weapons on the premises. Smoking is prohibited on the premises of a child care center as specified in Health and Safety Code Section 1596.795(b). A baby walker shall not be allowed on the premises of a child care center in accordance with Health and Safety Code Sections 1596.846(b) and (c). Licensees shall reveal each child care center license number in all advertisements in accordance with Health and Safety Code Section 1596.861

There are 8 cribs and 4 changing tables located in the infant center. LPA advised director that crib mattress shall be covered with a appropriate fitted sheet. Cribs shall not limit the ability of staff to see the infant, LPA observed a changing tables with a padded surface no less than one-inch thick, covered with washable vinyl or plastic, raised sides at least three inches high in good repair and safe condition. LPA observed changing table that are not within arms reach of a sink. Director was advised to remove or relocated changing tables that are not within arm’s reach of a sink.

Prior to the infant's first day at the center, the infant care center director or assistant director shall complete a needs and services plan for the infant. Bottles, dishes and containers of food brought by infants authorized representatives shall be labeled with infants name and current date.

LPA reviewed and provided applicant will copies and postings of Title 22 regulations related to infant safe sleep, , Infant Care Center Sleeping Equipment, Staff Ratios, Infant Needs and Service Plans,

Outdoor

The entire outdoor activity grounds are enclosed by a fence to protect children and to keep them in the outdoor activity area. The fence is at least four feet high. Areas around and under climbing equipment, swings, slides and other similar equipment has artificial grass and rubber platforms for cushioning to prevent injury from falls. The outdoor activity space was observed and maintained in good condition LPA did observe hazards in the outdoor activity area. There is adequate shade and benches for resting in the outdoor activity area. LPA did not observe any bodies of water.

The following items were discussed:

The director was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

LPA reviewed with facility representative the LIC 311A, Records To Be Maintained At The Facility, for child’s records, personnel records, administrative records, and documents to be posted.

Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test.

For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP).

LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP.
Child Care Center testing deadline has not past.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of “medication and equipment/supplies, and reviewed children’s, personnel, and
administrative records.

Decrease heated temperature of sink fixtures that disperse hot water and repair toilet in children's restroom with orange toilet cushion.
Place cushioning around spigots and wall mounts to prevent children injuries in play yard area
Relocate or remove changing tables that are not within arm's reach of a sink
Post required documents in an area visible to parents and visitors to review
Update facility sketch

Corrections are due by 07/10/2025.

An exit interview was conducted with plan of corrected items discussed. LPA provided Alyssa Lao with appeal rights and a copy of this report. Final license determination will be made upon review by the Licensing Program Manager and corrections.
