On 2/5/2025 Licensing Program Analysts (LPA) Laticia Thompson met with Angela De Silva, Director. The purpose of the visit was an announced pre-licensing inspection at the facility to ensure that health, safety and personal rights as required by Title 22 and Health and Safety Regulations governing California Child Care Centers will be met. There were no children present during this inspection. This is an emergency relocation application for 61 children ages 2-6 years. The child care center will operate Monday – Friday 7:30 a.m. – 5:00 p.m. The fire clearance was approved on 1/23/2025. LPA toured and measured the on-limits areas. The facility has a total of 4 classrooms. Children will have access to 5 sinks and 5 toilets.

The Indoor measurements contain a total of 2,159.17 which will accommodate Applicant’s request for 61 children.

Outdoor Play measurements are:

The outdoor activity space measurements contain a total of 4,803.81 which will accommodate Applicant’s request for 61 children.

Parents will sign in and out electronically under the BriteWheel application. LPA advised facility there should be a plan in place on how such records will be immediately printable upon request for inspection and audit by program staff. For best practice purposes, facilities should have a plan in place on how records would be backed up, and how they would be made available in the event of a power failure or disaster. LPA reference director to review PIN 18-03.1-CCP.

The facility will provide filtered water through dispensers or pitchers where children can fill their personal water bottles for outdoor and indoor activities. There are operable carbon monoxide detectors located in classrooms 202, 203 and 204. LPA observed fire extinguishers with an inspection date of 07/12/2024. The First Aid Kits were not equipped with the required materials.

Furniture and equipment were observed to be maintained in good condition free of sharp, loose or pointed parts. Tables and chairs were observed to meet the needs of the children. The facility is not equipped with cots or mats for napping. There is a sufficient quantity and variety of age- appropriate play equipment in good condition. There are no cubbies for storage of children clothing and personal belongings. Classroom 202 and the Wesley room has central air for heating and windows for ventilation. Classrooms 203 and 204 have individual units for heating and cooling. The classrooms were clean in good repair. Classrooms have adequate lighting. Windows were in good repair free of chipping paint, dirt, insects, or debris. LPA observed trash cans with tight fitting lids. The facility will be equipped with a cellphone pending landline telephones service installation. Disinfectants, cleaning solutions, and other items that are dangerous to children are stored in a storage room inaccessible to children.

All toilets, hand washing and bathing facilities are maintained in safe and sanitary operating condition. Hot water temperature controls are maintained within regulation.

The applicant shall be responsible for ensuring that children with obvious symptoms of illness are not accepted. The isolation area for Ill children will be located in the Wesley Room . The isolation area shall be equipped with a cot for ill children and shall be supervised by staff. The staff restroom is designated for children who are ill.
The applicant was advised to develop and implement a written plan to record the administration of prescription and nonprescription medications, to inform the child's authorized representative daily when such medications have been given. Medication will be stored in the kitchen inaccessible to children. Medication that requires refrigeration will be stored in the refrigerator located in the kitchen inaccessible to children.

The program provides snacks. The kitchen and food-preparation storage areas were observed to be clean and free of litter and rubbish. All foods or beverages that require storage at a temperature of 45 degrees will be stored in the kitchen refrigerator. The refrigerator isn’t equipped with a thermostat to regulate temperature control. Menus are in writing and are posted in advance in an area accessible for review by the child's authorized representative. Copies of the menus as served shall be dated and kept on file for at least 30 days. Menus shall be made available for review by the child's authorized representative and the Department upon request.

The required postings are posted in an area for parents and visitors to review. Facility does not have a disaster and mass casualty plan of action. The plan shall be in writing and shall be readily available with contingency plans for action during fires, floods and earthquakes.

The facility does not provide transportation. Firearms and other weapons shall not be allowed on or stored on the premises of a child care. Director stated there aren’t any firearms or weapons on the premises. Smoking is prohibited on the premises of a child care center as specified in Health and Safety Code Section 1596.795(b). Licensees shall reveal each child care center license number in all advertisements in accordance with Health and Safety Code Section 1596.861.

Outdoor

The entire outdoor activity grounds are enclosed by a fence to protect children and to keep them in the outdoor activity area. The fence is at least four feet high. Areas around and under climbing equipment, swings, slides and other similar equipment has sand beneath for cushioning to prevent injury from falls. The outdoor activity space was observed and maintained in good condition LPA did observe hazards in the outdoor activity area. There is adequate shade and benches for resting in the outdoor activity area. Applicant was advised to inspect sandboxes daily to ensure areas are free of any hazardous foreign material. Director denies that there is any bodies of water on the premises. LPA did not observe any bodies of water.

The following items were discussed:



The director and applicant were reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

LPA reviewed with facility representative the LIC 311A, Records To Be Maintained At The Facility, for child’s records, personnel records, administrative records, and documents to be posted.

Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test.

For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP).

The applicant and director was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

LPA discussed the safe sleep regulations with Director and discussed the Child Care Licensing Safe Sleep webpage at: https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep, as an additional resource. LPA also informed [applicant, licensee, or facility representative] of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at: https://www.cpsc.gov/, and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

The following corrections are needed

Facility will provide pictures of storage/cubbies for children personal belongings and clothes.
Facility will provide pictures of Wesley room with age-appropriate furniture, play equipment, toys and fixtures.
Facility has placed an order for napping equipment (cots/mats) pending delivery.
Facility will purchase a first aid kit with a manual and sterile dressing.

Facility will purchase a thermometer.
Facility shall make adjustment to unleveled cement in the outdoor activity space.
Facility shall make adjustments to water spigots and other hazardous located in the outdoor activity space.
Facility will place a thermostat in kitchen refrigerator to regulate temperature.

Facility shall develop a Mass Casualty and Disaster Plan.
Facility will submit a copy of LIC 610 and updated facility sketch to Regional Office.

An exit interview was conducted with plan of corrected items discussed. LPA provided Director with appeal rights and a copy of this report. Final license determination will be made upon review by the Licensing Program Manager and corrections.
