On 12/10/2025 Licensing Program Analysts (LPA) Laticia Thompson, Tatiana Bickham and Melissa Zaragoza met with Charlene Barkes,Director. The purpose of the visit was an announced pre-licensing inspection at the facility to ensure that health, safety and personal rights as required by Title 22 and Health and Safety Regulations governing California Child Care will be met. There were 82 children present during this inspection. This is an application for a Single License Conversion (Previously licensed under # 197403964) with a Total capacity of 151; 12 Toddlers ages 18 and 36 months and 139 Preschoolers ages 2 – 6 years. The child care center will operate Monday – Friday 7:00 a.m. – 6:00 p.m. LPA's toured and measured the on-limits areas. The fire clearance was approved on 04/02/2025. The facility has a total of 10 classrooms. Classroom #3 will be dedicated to the Toddler program and classrooms 1,2, 4-10 will be dedicated to the preschool. Toddlers & Preschool children will have access to 12 sinks, 10 toilets and 2 Urinals.

Toddler Indoor measurements are:

The measurements contain a total of 431.84 square feet which will accommodate Applicant’s request for 12 Toddlers.

Preschool Indoor measurements are:

The measurements contain a total of 5003.60 square feet which will accommodate Applicant’s request for 139 Preschool children.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

Toddler Outdoor Activity measurements are:

The outdoor activity space measurements contain a total of 2,210.00 square feet which will accommodate Applicant’s request for 12 toddlers.

Preschool Outdoor Activity measurements are:

The outdoor activity space measurements contain a total of 11,954.00 square feet which will accommodate Applicant’s request for 139 Preschool children.

The following was observed:



Parents will sign children in and out manually on the sign in and out sheets located at the front entrance of the school

Staff restrooms are located in the main office and next to classroom #10.

The facility has filtered water dispensers where children can fill their personal water bottles for outdoor and indoor activities. There are operable carbon monoxide detectors in all classrooms LPA observed fire extinguishers with an inspection dates 01/07/2025. LPA observed First Aid Kits with the required materials including a thermometer.

Furniture and equipment were observed to be maintained in good condition free of sharp, loose or pointed parts. Tables and chairs were observed to meet the needs of the children. There are mats for napping. Director was advised to replace yoga mats with sleeping mats that meet regulation requirements. LPA confirmed that mats are colored so that the sleeping side (red) can be distinguished from the floor side (blue). There is sufficient quantity and variety of age- appropriate play equipment in good condition. There are cubbies and hanging hooks for storage of children clothing and personal belongings. The classrooms have central air for heating and cooling. The classrooms were clean in good repair. Classrooms have adequate lighting. Windows seals are in need of cleaning by removal of dust and insects. LPA advised director to replace trash cans that are in need of lids. The facility is equipped with landline telephones service.

Disinfectants, cleaning solutions, and other items that are dangerous to children are stored on highly leveled shelves and in the janitors closet inaccessible to children.

All toilets and hand washing facilities are maintained in safe and sanitary operating condition. Hot water temperature controls are maintained within regulation.

The licensee shall be responsible for ensuring that children with obvious symptoms of illness are not accepted. The isolation area for Ill children is located in the director’s office. The isolation area shall be equipped with a cot for ill children and shall be supervised by staff. The staff restroom located in the office is designated for children who are ill.



The facility has developed and implemented a written plan to record the administration of prescription and nonprescription medications, to inform the child's authorized representative daily when such medications have been given. Medication that requires refrigeration is stored in the refrigerator located in the kitchen and other medications in a cabinet inaccessible to children.

The program provides snacks only. The kitchen and food-preparation storage areas were observed to be clean and free of litter and rubbish. All foods or beverages that require storage at a temperature of 45 degrees are stored in the refrigerators located in both kitchens. The refrigerators are equipped with a thermostat to regulate temperature control Menus are in writing and are posted in advance in an area accessible for review by the child's authorized representative. Copies of the menus as served shall be dated and kept on file for at least 30 days. Menus shall be made available for review by the child's authorized representative and the Department upon request.

The required postings are posted in front of the main office for parents and visitors to review. Facility has a disaster and mass casualty plan of action. The plan is in writing and readily available with contingency plans for action during fires, floods and earthquakes.

The facility does not provide transportation. Firearms and other weapons shall not be allowed on or stored on the premises of a child care. The director stated there aren’t any firearms or weapons on the premises. Smoking is prohibited on the premises of a child care center as specified in Health and Safety Code Section 1596.795(b). The Director will ensure that the child care center license number is provided on all advertisements in accordance with Health and Safety Code Section 1596.861
LPA advised applicant of the following regarding the toddler Program:

101216.4(a)(1) Preschool Program with Toddler Component A child who is between 18 months and 36 months of age may participate in the toddler program with written permission from the child's authorized representative. No child in the toddler program shall be placed in the preschool program before the age of 30 months without written permission from the child's authorized representative.

Outdoor

The entire outdoor activity grounds are enclosed by a fence to protect children and to keep them in the outdoor activity area. The fence is at least four feet high. Play equipment and materials used by children are age-appropriate. Areas around and under climbing equipment, swings, slides and other similar equipment has chips and artificial grass for cushioning to prevent injury from falls. The outdoor activity space was observed and maintained in good condition LPA observe a rose bush with thorns that can pose a hazard in the toddler outdoor activity area. There is adequate shade and benches for resting in the outdoor activity area. Applicant was advised to inspect sandboxes daily to ensure areas are free of any hazardous foreign material. LPA did not observe any bodies of water.

The following items were discussed:


The director was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.
