On 02/10/2026 Licensing Program Analysts (LPA) Laticia Thompson met with Maria Martinez, Applicant/Director. The purpose of the visit was an announced pre-licensing inspection at the facility to ensure that health, safety and personal rights as required by Title 22 and Health and Safety Regulations governing California Child Care Centers will be met. The child care facility is located on the grounds of South Hills Church. There were no children present during this inspection. This is an application for a Single License with a Total capacity of 15 Preschool children ages 2 -6years of age. The child care center will operate Monday – Friday 6:00 a.m. – 6:00 p.m. LPA toured and measured the on-limits areas. The fire clearance was approved on 8/21/2025. The facility will be licensed for 1 classroom. Preschool children will have access to 1 sink and 1 toilet. The church grounds and preschool are equipped with surveillance cameras.

Preschool Indoor measurements

The measurements contain a total of 500.66 square feet which will not accommodate Applicant’s request for 15 Preschool children. Based on today’s measurement the facility capacity limit for indoor space is 14 Preschool children.

The outdoor activity space measurements contain a total of 1,288.70 square feet, which will accommodate Applicant’s request for 15 Preschool children.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

The following was observed:

Parents will enter and exit through the doors of the building located on Victory Blvd. Parents will sign children in and out manually on the sign in and out sheets located in the classroom.

The Staff restroom is located across the hall from the children classroom.



The facility has a filtered water dispenser where staff will fill children personal water bottles for outdoor and indoor activities. There is an operable carbon monoxide detector located in the classroom. LPA observed a fire extinguisher with an inspection date of 10/15/2025. LPA observed First Aid Kits with the required materials per regulations

Furniture and equipment were observed to be maintained in good condition free of sharp, loose or pointed parts. Tables and chairs were observed to meet the needs of the children. There is sufficient quantity and variety of age- appropriate play equipment along with cots for napping in good condition. There are cubbies for storage of children clothing and personal belongings. The classroom has central air for heating and cooling. The classroom is clean in good repair having adequate lighting. The windows are in good repair free of chipping paint, dirt, insects, or debris. LPA observed trash cans with tight fitting lids. The facility will be equipped with landline telephones service. Disinfectants, cleaning solutions, and other items that are dangerous to children are stored in the children’s restroom cabinet with a safety latch making items inaccessible to children.

All toilets and handwashing facilities are maintained in safe and sanitary operating condition. Handwashing fixtures do not deliver hot water.

The director shall be responsible for ensuring that children with obvious symptoms of illness are not accepted. The isolation area for Ill children will be located in the director’s office and the restroom across from the director’s office. The isolation area shall be equipped with a cot for ill children and shall be supervised by staff.

The applicant was advised to develop and implement a written plan to record the administration of prescription and nonprescription medications, to inform the child's authorized representative daily when such medications have been given. Medication will be stored in a locked box in the directors office inaccessible to children. Medication that requires refrigeration will be stored in the refrigerator located in the directors office with a safety strap inaccessible to children.

The program provides snacks. The kitchen and food-preparation storage areas were observed to be clean and free of litter and rubbish. All foods or beverages that require storage at a temperature of 45 degrees will be stored in the refrigerator located in the church kitchen and directors office. The refrigerator located in the church kitchen is equipped with a thermostat to regulate temperature control Menus are in writing and are posted in advance in an area accessible for review by the child's authorized representative. Copies of the menus as served shall be dated and kept on file for at least 30 days. Menus shall be made available for review by the child's authorized representative and the Department upon request.

The required postings are posted on a moveable board in the entrance of the lobby area for parents and visitors to review. Facility does not have a disaster and mass casualty plan of action. The director will create a plan in writing and shall be readily available with contingency plans for action during fires, floods and earthquakes.

The facility does not provide transportation. Firearms and other weapons shall not be allowed on or stored on the premises of a child care. The director stated there aren’t any firearms or weapons on the premises. Smoking is prohibited on the premises of a child care center as specified in Health and Safety Code Section 1596.795(b). Applicant/Director was advised that Licensees shall reveal each child care center license number in all advertisements, such as banners, websites, business cards etc in accordance with Health and Safety Code Section 1596.861

The entire outdoor activity grounds are enclosed by a fence to protect children and to keep them in the outdoor activity area. The fence is at least four feet high. Play equipment and materials used by children are age-appropriate. Areas around and under climbing equipment, swings, slides and other similar equipment has artificial grass for cushioning to prevent injury from falls. The outdoor activity space was observed and maintained in good condition LPA observed some hazards in the outdoor activity area (see plan of corrections). There is adequate shade and benches for resting in the outdoor activity areal. LPA did not observe any bodies of water.

The director was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

LPA reviewed with applicant the LIC 311A, Records to be Maintained at the Facility, for child’s records, personnel records, administrative records, and documents to be posted. Entrance Checklist was provided to the applicant.
LPA discussed the safe sleep regulations with [applicant, licensee, or facility representative] and discussed the Child Care Licensing Safe Sleep webpage at: https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep, as an additional resource. Applicant was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

The following corrections are needed before licensure will be considered.
Provide a copy of Disaster and Mass Casualty Plan
Proof of landline telephone service.
Repair loose wooden panel and gap between fencing in the outdoor activity space. Install cushioning around water spigot and repair water leak.
Install a temperature regulator in refrigerator in directors office.
LPA recommended that facility install an alarm on the double glass doors that leads to outside of the building next to the classroom and the gate that leads to parking lot in the outdoor activity area.
Submit documents requested on Notice of Incomplete Application.
Corrections are due by 03/11/2026

An exit interview was conducted with plan of corrected items discussed. LPA provided Maria Martinez with appeal rights and a copy of this report. Final license determination will be made upon review by the Licensing Program Manager and corrections.
