On 8/14/2026, Licensing Program Analyst (LPA) Amelia Morales conducted an unannounced- Required Annual inspection to the above facility. The purpose of the inspection was to ensure that health, safety, and personal rights as required by Title 22 Regulations governing California Family Child Care Homes are being met by the Licensee. LPA Morales was greeted by Licensee Vahagn Avetisyan and Licensees Spouse Nelli Voskanyan, the facility has been licensed for Small (8) Family Child Care home license since 8/12/2026. At the time of the visit Sirush Avetisyan provided Armenian interpretation and exited facility at 9:30 AM.

The License is applying for an increase of capacity; Large Family Child Care Home license for a maximum capacity of 14. The Fire clearance was granted on 7/28/2026 LA City Fire Prevention Bureau. It should be noted facility refers themselves as "Little Universe Child Care."

An entrance checklist was provided. A census was taken there were 2 children during the time of the visit. The children’s roster was reviewed and is current. Per Licensees, the facilities hours of operation are Monday through Friday from 7:00 AM to 10:00 PM

It should be noted that the facility is part of duplex property located in the backyard, with other property separate address (6927 Bellaire Ave. North Hollywood, 91605 CA.). The other property has a different entry way and is off limits to the children in care.

This is a single dwelling home which consist of: front yard (fenced), 3 bedroom, 2 bathroom, living room, kitchen, laundry room, backyard (fenced). At the entrance of the home there is a visible board to display required information.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

Per Licensee, areas that are accessible to children include; bedroom #1 which serves as the napping area for children. LPA observed each child to have their own individual cot. LPA observed playpen empty and free of any loose article. LPA observed 15 min sleep logs for infant children. Per license, they provide sheets and blankets for enrolled children. Per license, sheets and blankets are laundered weekly by parents.

Additional areas that are accessible to children include the living room, bathroom #1, and front yard (fenced). LPA observed that the outdoor play area is fenced. Per Licensee, they visually supervise children at all times and never leave children unattended. The children eat in the living room. The fireplace located in the living room is made inaccessible and is barricaded. The combined smoke detector/carbon monoxide detector was observed in the living room and in bedroom #1, was tested and found to be operable.

Per Licensee, areas off limits to children and parents include: bedroom #2, bedroom #3, bathroom #2 located inside bedroom #3 and laundry room. LPA observed door level locks making the rooms inaccessible to the children in care. The kitchen is off limits and is made inaccessible by a retractable baby gate. The kitchen has a child safety locks making hazardous items inaccessible to the children in care. Per Licensee, food is provided such as breakfast, lunch, snacks and dinner. The backyard is off limits to the children in care.

Home does have a swimming pool, located in the backyard that is fenced. The fence is at least 5 feet high with a self-latching gate that opens away from the pool or body of water. LPA observed a rescue pole with a body hook and minimum fixed length of 12 feet, LPA observed a life ring with a minimum exterior diameter of 17 inches and a pool alarm. The pool alarm was tested and found to be operable. LPA observed a pool alarm that is ASTM International Standard F2208, approved. Alarm was tested and found to be operable. LPA observed a daily log where they are documenting daily pool inspections ensuring pool safety and compliance.

LPA observed the following: the home is clean and orderly. There were age-appropriate toys and play items. Licensees were informed that baby-walkers, bouncers, jumpers, and other prohibited items will not be used for children in care. All electrical outlets have safety covers.

Detergents, cleaning compounds, medications, and other items which can pose a danger to children are inaccessible. The Licensee states that there are no poisons in the home. The Licensee does understand that poison must be locked with a key or combination lock. LPA did not observe a pet at the time of visit.  Per Licensee, isolation area for sick children is in bedroom #1. Per Licensee there are no firearms on the premise.

LPA Morales observed the required (2A10BC) fire extinguisher was mounted and located in the entrance of the home. LPA reminded Licensee that a required (2A10BC) fire extinguisher must be purchased or charged yearly. The last emergency disaster drill was done 7/24/2026. LPA observed a completed first aid kit located in the kitchen cabinet. LPA reviewed Staff and Children’s records for completeness. Inspection of required forms was made and documented on LIC857 for children and the LIC859 for staff.

LPA reviewed children's records, which were complete. LPA reviewed the licensees records and assistants, which were complete. The Licensee has current Pediatric First Aid and CPR. Proof of immunization against influenza, pertussis, and measles was readily available during today’s inspection. The Licensees has also taken the Mandated Reporter Training.

LPA reminded Licensee that the Mandated Reporter Certification and Pediatric First Aid and CPR must be renewed every two years. LPA also reminded Licensee that Pediatric First Aid and CPR must be taken in person.

All individuals residing in the home were noted, discussed, and staff working with children have criminal record clearance on file.

Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

Because the licensees rents/leases the home, proof of landlord notification is required.  The LPA observed the Property Owner/Landlord Notification form (LIC9151) that the applicant confirms was provided to the property owner/landlord. The Licensee obtained a signed Property Owner/Landlord Consent form (LIC 9149).

LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep as an additional resource. LPA also informed licensee [or facility representative] of the importance of checking for and removing any recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

LPA Morales informed Licensee to never accept a sleeping child in the car seat. LPA informed licensee that there is one crib or play yard for each infant in care, cribs and play yards are kept free from all loose articles and objects while infants are sleeping, and there are no objects hanging above or attached to the crib or play yard. Infants are not swaddled while in care.

Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22- 02-CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. During the exit interview, the Licensee Vahagn Avetisyan, confirmed that there are no Registered Sex Offenders living in the facility and LPA completed the RSO profile in FAS.

Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.
