On 5/15/2026, Licensing Program Analyst (LPA) Samantha Barba and Licensing Program Manager (LPM) Emiko Bell arrived at Facility #195700760 Mazmanyan Family Child Care Home which is a relocation and was previously licensed as 195700197. LPA Barba and LPM Bell arrived at the family childcare home at 8:50 am and met with applicant Narine Mazmanyan and husband Armen Begoyan to which they announced the purpose of the visit. This was a pre-licensing visit to ensure that health, safety, and personal rights as required by Title 22 and Health and Safety Regulations governing California Child Care Homes will be met. This is an application for a large family Child Care Home. The home is a one-story home that consists of 4 bedrooms, 3 bathrooms, living room, dining room, kitchen, attached garage, front yard and back yard. Applicant Narine Mazmanyan provided proof of control of property / grant deed which shows ownership of the home located at 9054 Darby Ave, Northridge CA 91325.

Applicant is requesting a capacity of 14 children, ages 0 months to 13 years old. The Family Child Care Home will operate Monday – Friday 7:00 AM – 10:00 PM. Entrance check list was provided, and a detailed tour of the home was given. Applicant stated that a cell phone, with active service in the home will be the main contact number while children are in care. The home was granted clearance on 05/05/26 by the fire department for a large capacity home which had the pull fire alarm and exit signs. There was a rechargeable fire extinguisher in the home 2A-10BC located on the wall in the living room/ dining room area which was observed to be in good condition and purchased on 05/15/2026. There was a carbon monoxide detector in the hallway near the bedrooms and a smoke detector observed in the entrance of the home both tested and are in working condition.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

Bedrooms #1, #2, #3 and #4, bathrooms #1 and #2 were all located on the right side of the entrance which are off limits. The hallway to that area of the home is rendered inaccessible to the day care with a latched gate. All bedrooms and bathrooms were inspected. At the time of the announced visit, the home was occupied by the applicants 2 children under the age of 18, and 3 adults which all live in the home, the applicant Narine, her husband and daughter who all have Criminal Background Clearance. Applicant was reminded that all adults 18 and over living in the home, persons who provide care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.

The living room and dining areas were converted to the day care space which were observed to have child- sized furniture, toys, cots, and one infant crib as well as latched cupboards for safety. The kitchen is an on limits area due to the children passing through to enter bathroom #3, which is on limits to the children in care. All cleaning supplies and potential hazards to children including knives and scissors were observed to be placed in locked cabinets or high areas that were out of reach of children and were rendered inaccessible.

The backyard is an on-limit area which is accessible through the day care areas (dining room and living room converted to day care only area) via 2 doors with locks. The backyard was observed to have appropriate toys and fixtures along with it being clear and tidy.

Applicant was informed that it is required by law to post the following in the facility:

Emergency Disaster Plan (LIC 610A - This must be posted in your home the Earthquake Preparedness Checklist (LIC 9148), must be attached to the LIC 610A and available to the public.

Notification of Parents' Rights Poster (PUB 394) - This poster must be placed in an area of the home where all parents can see it

Facility License, (LIC 203)-Your Family Child Care Home License must be posted in an area of the home where it can be easily seen by parents and caretakers.

Notice of Site Visit (LIC 9213) must remain posted for 30 days (during the hours that children are in care) after each site visit by a licensing representative.

Any licensing report documenting a type “A” citation must be posted for 30 days during the hours that children are in care.

Any licensing report or other document verifying compliance or non-compliance with the Department’s order to correct a Type “A” deficiency must be posted for 30 days during the hours that children are in care.

Employee records must be maintained by the applicant and shall contain the following: LIC9052/Employee Rights, LIC 9108 Mandated Reporter Form, An employment contract or application containing the following information: Employees Full Name, Date of Birth, Driver's License Number, Date of Employment, Documentation of Health and Safety Training, Duties of the employee. Licensee is reminded that records for employees as well as children must be maintained for 3 years after separation for the Family Child Care Home( FCCH).

Mandatory Forms for the children’s files: (LIC 282 Affidavit Liability Insurance, LIC 627/Consent for Medical Treatment, LIC 700/ID and Emergency Information, LIC 995A/Parent's Rights, LIC995E/Caregiver Background Check, LIC 9150/Parent Notification, LIC 9212/Parent's Responsibilities, PM 286/Immunization (Blue) Card).

Requirements for fire drills, earthquake drills, and documentation for both. Role and responsibilities of being a mandated reporter were reviewed. The applicant was advised how to access forms and Regulations for Family Child Care online at www.ccld.ca.gov. Regulation prohibits the smoking of tobacco in a private residence licensed as a family childcare home during the hours of operation. State law prohibits baby walkers, bouncy seats, exersaucers and any other items that fall into that category.

LPA discussed the safe sleep regulations with applicant and discussed the Child Care Licensing Safe Sleep webpage at: https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep, as an additional resource. LPA also informed [applicant, licensee, or facility representative] of the importance of checking for and removing recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at: https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

On this date, 05/14/2026, the California Attorney General - Megan’s Law website www.meganslaw.ca.gov was searched for information on sex offenders required to register with local law enforcement under California's Megan's Law. No registered sex offenders were found at the facility address. Under state law, some registered sex offenders are not subject to public disclosure; therefore, they may not have been included in this search. However, the Department conducts a monthly cross reference of each address on record for all registered sex offenders against all CCLD facility addresses pursuant to information shared by California DOJ. Home was found to be clear of any Registered Sex Offenders.

Applicant was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

The licensee was informed of The Child Care Advocate Program (CCAP) that is administered from within the Community Care Licensing Division.
