32 | Applicant has proof of immunization against pertussis and measles, and declination letters for influenza on file. Proof of Mandated Reporter Training was also provided, however, expired 5/27/2026. Applicant will renew Mandated Reporter Training and submit to LPA for review.
Applicant states will provide meals and snacks for children. There are currently few toys in the home as applicant is still running their original FCCH location. Advised applicant that walkers, jumpers, rockers, exersaucer are not prohibited. The required (2A10BC) fire extinguisher is in the laundry room off the foyer. Per Applicant, there are no weapons or firearms in the facility.
Per Applicant, the children will use the sideyard for outdoor play. The outdoor play area was observed to be fenced. LPA observed no hazards in the children’s designated outside play area. This is a duplex property and the back unit has a separate address (7038 Tyrone Ave). Licensee states her mother (Guardian cleared adult) will be renting the back unit and she is considering opening an after school care FCCH. LPA advised licensee that she can do that, but must not co-mingle children, licensee understood. There are no pools or spas, or other bodies of water. The isolation area for sick children waiting to be picked up will be in the dining area (unless in use), away from other children. The Applicant provided proof of control of property, mortgage documents provided.
Applicant was reminded that all adults 18 and over living in the home, persons who provide care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.
LPA informed Applicant any unusual incidents or injuries must be reported to the Department within 24 hours via telephone and within seven (7) days in writing. Fire and safety drills must be performed every six months and documented for review by the Department and smoking tobacco is not allowed in a home that is licensed as a family day care home, and in those areas of the family day care home where children are present.
A current roster of children enrolled must be available and maintained for a period of three years, even after children no longer are attending the facility. Annual fees must be paid promptly and by the due date or a late fee shall be assessed, and/or the License shall be terminated.
