On 07/28/2026 Licensing Program Analysts (LPA’s) Laticia Thompson and Nathanael Mooberry met with OBIOMA UCHE, ECE Director. The purpose of the visit was an announced pre-licensing inspection at the facility to ensure that health, safety and personal rights as required by Title 22 and Health and Safety Regulations governing California Child Care Centers will be met. There were no children present during this inspection. This is an application for a Single License with a total of 24 Preschoolers ages 2- 5years. The child care center will operate Monday – Friday 7:45 a.m. – 6:00 p.m. LPA's toured and measured the on-limits areas. The fire clearance was approved on 05/13/2026. The facility has 1 classroom. The Preschool is located on Los Angeles Unified School District l grounds. Classroom #5 will be used for the Preschool children. Children will have access to 2 sinks and 2 toilets located in the classroom.

Preschool Indoor measurements are:

The measurements contain a total of 967.87 square feet which will accommodate Applicant’s request for 24 Preschool children.

Preschool Outdoor Activity measurements are:

The outdoor activity space measurements contain a total of 4,934.61 square feet, which will accommodate Applicant’s request.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

The following was observed:

Parents will enter and exit through a gate located on the preschool outdoor activity area, where children will be signed in and out manually.

Per Health and Safety Code 1596.841 Each child day care facility shall maintain a current roster of children who are provided care in the facility. The roster shall include the name, address, and daytime telephone number of the child's parent or guardian, and the name and telephone number of the child's physician. This roster shall be available to the licensing agency upon request.



Staff restrooms are located on the Elementary school grounds separated from preschool children . The facility will provide bottled water in addition staff will fill children personal water bottles by using a water faucet located across from the class room and a filtered water dispenser located on the elementary school playground. There is an operable carbon monoxide detector located in the classroom. LPA observed a fire extinguisher in the hall across from the classroom with an inspection date of 5/1/2026. The facility has a First Aid Kit with all required material and a thermometer located in the nurses office.

Furniture and equipment were observed to be maintained in good condition free of sharp, loose or pointed parts. Tables and chairs were observed to meet the needs of the children. There are 20 cots for napping maintained in safe condition. LPA confirmed that There is sufficient quantity and variety of age- appropriate play equipment in good condition. There are cubbies in all classrooms for storage of children clothing and personal belongings. The classroom has central air for heating and cooling. The classrooms were clean in good repair. Classrooms have adequate lighting. Windows were in good repair free of chipping paint, dirt, insects, or debris. LPA observed trash cans with tight fitting lids. The facility is equipped with landline telephones service reachable to and from the main office and other classrooms. Disinfectants, cleaning solutions, and other items that are dangerous to children are stored the hopper room inaccessible to children.

All toilets and handwashing facilities are maintained in safe and sanitary operating condition. Handwashing fixtures do not deliver hot water.

The licensee shall be responsible for ensuring that children with obvious symptoms of illness are not accepted. The isolation area and restroom for Ill children will be located in the nurses office. The isolation area shall be equipped with a cot for ill children and shall be supervised by staff.



The licensee was advised to develop and implement a written plan to record the administration of prescription and nonprescription medications, to inform the child's authorized representative daily when such medications have been given. Medication will be stored in the nurses office inaccessible to children. Medication that requires refrigeration will be stored in the refrigerator located in the nurses office.

The program provides breakfast lunch and snacks. The kitchen and food-preparation storage areas were observed to be clean and free of litter and rubbish. Foods and beverages that require storage at a temperature of 45 degrees are stored in the refrigerator located in the schools cafeteria. Menus are in writing and are posted in advance in an area accessible for review by the child's authorized representative. Copies of the menus as served shall be dated and kept on file for at least 30 days. Menus shall be made available for review by the child's authorized representative and the Department upon request.

The required postings are posted in the classroom for parents and visitors to review. Facility has a disaster and mass casualty plan of action. The plan is in writing and readily available with contingency plans for action during fires, floods and earthquakes.

The facility does not provide transportation. Firearms and other weapons shall not be allowed on or stored on the premises of a child care. The Director stated there aren’t any firearms or weapons on the premises. Smoking is prohibited on the premises of a child care center as specified in Health and Safety Code Section 1596.795(b). Licensees shall reveal each child care center license number in all advertisements in accordance with Health and Safety Code Section 1596.861

Outdoor

The entire outdoor activity grounds are enclosed by a fence to protect children and to keep them in the outdoor activity area. The fence is at least four feet high. Play equipment and materials used by children are age-appropriate. Areas around and under climbing equipment, swings, slides and other similar equipment has rubber platforms for cushioning to prevent injury from falls. The outdoor activity space was observed and maintained in good condition LPA observed hazards (see plan of correction) in the outdoor activity area. There is adequate shade and benches for resting in the outdoor activity area. LPA did not observe any bodies of water and Director confirmed.

The following items were discussed:



The director was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center.

Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for
drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test.

For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP).

CCC Testing deadline has not passed.

Applicant was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

  • FACIITY WILL PROVIDE PICTURES OF RESOLVED TRIPPING HAZARD WITH SPRINKLER WITHIN THE DIRT AREA, REPAIR LEVELED DIRT AND CEMENT PATHS.
