25 | Licensing Program Analyst (LPA) Emily Peraldi conducted an unannounced case management visit at the above location. At 9:52 a.m., the LPA met with staff and explained the reason for the visit. At 10:06 a.m., the Administrator Erna Gevorgyan arrived at the facility.
On 02/21/2024, a Notice of Operation in Violation of Law (NOVL) was issued due to the Licensee selling the business prior to the change of ownership application completion and issuance of a new license. The Applicants explained that an application for a change of ownership was sent out on January 2024. During today’s visit, the LPA had a conversation with the Administrator with the goal of providing education on the transferability of a license. The LPA explained that per Title 22, Division 6 Chapter 8, 87109 Transferability of License- (a)The license shall not be transferable. The LPA also provided education on Health and Safety Code 1569.191 Sale of licensed facility; resulting issuance of new license; procedure. The Administrator stated that they understand that the business should not have been sold without the proper procedures change of ownership. At this time, the change of ownership application is still in process. At 10:13 a.m., the LPA called the previous Licensee, Davit Hakobyan, and left a message for a call back. The LPA reminded the Administrator to not take in additional residents.
The purpose of the visit is to ensure there are no health and safety hazards. At 10:15 a.m., the LPA along with Administrator toured the facility. The LPA observed four (4) residents at this location.
No deficiencies issued. Exit interview conducted. A copy of the report was provided. |
