25 | On 5/02/2022, Licensing Program Analyst (LPA), Sandra Urena conducted a subsequent pre-licensing visit. The LPA arrived at the facility at 11:04 a.m., and met with applicant Elba Kachatryan. This is a new facility application for five non-ambulatory residents, and one bedridden resident.
On 04/14/2022, at 11:00 a.m.,the initial pre-licensing visit was conducted by Licensing Program Analyst (LPA), Sandra Urena. The LPA arrived at the facility at 10:30 a.m., and met with applicant Elba Kachatryan. This is a new facility application for five non ambulatory residents and one bedridden resident. The LPA, and the applicant toured the physical plant areas inside and outside to ensure there are no health and safety hazards and facility is in compliance with Title 22 Regulations.
At 11:30 a.m., the LPA and the applicant toured the facility to observe the corrections needed and noted on the initial pre-licensing visit conducted on 04/14/2022. The following corrections were observed.
Ramp was installed to give access to the backyard to all residents.
Emergency water was observed to be a seven-day supply for residents and staff.
Hand washing signs are posted in bathrooms.
Auditory alarm was installed in bedroom # 3
Bottom threshold from doors leading from the hallway to kitchen and common area, were cleared to the level where wheelchairs and walkers may pass through safely.
