Licensing Program Analyst (LPA) Quoc Huynh conducted a pre-licensing visit to the above noted facility. At 10:12AM, the LPA met with Applicant Movses Movsesyan. Entrance interview conducted.

An application to operate a Residential Care Facility for the Elderly was submitted on 10/17/2025. A dementia program was included in the plan of operation. There is a pending Hospice Waiver for four (4). The facility's fire clearance was approved on 04/02/2026 for two (2) ambulatory, one (1) non-ambulatory, and three (3) bedridden for a total of six (6) residents. Component II orientation was completed on 05/20/2026. Component III was completed with the Applicant during today’s visit. Beginning at 10:15AM, a physical plant tour was conducted. The following was observed:

OUTDOOR AREA: The exterior passageways were clean and clear of any obstructions. The front patio had furniture in good condition. The rear patio will also be a driveway and had a driveway gate. There was a total of two (2) emergency exits. There were no bodies of water on the premises at the present time.

COMMON AREAS: The living room was appropriately furnished, and the lighting was adequate. There was a television and a desk. A dining room table was observed with sufficient seating. There was sufficient space to accommodate both indoor and outdoor activities.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

The physical plant is consistent with the submitted facility sketch. The facility had emergency lighting, which included but not limited to flashlights. Nightlights were observed throughout the common areas. The facility has central heating and air conditioning to maintain rooms to a comfortable temperature. Hardwired combination smoke and carbon monoxide detectors were tested at 10:43AM and operational. The fire extinguisher was fully charged and purchased on 03/06/2026. A supply of extra linens, supplies, and medication storage was observed in a locked cabinet. There was a secured laundry closet with machines in good condition and detergent. There is a functioning telephone on the premises. All required postings are posted near the entrance of the facility. There is a storage closet that contained extra supplies, emergency water, and facility file cabinets.

KITCHEN: Knives and cleaning supplies were secured. The supply of dishes, utensils, pots, pans and drinkware is adequate. The freezer was maintained at 0*F and the refrigerator was maintained at 40*F. The supply of nonperishable foods and emergency food supply is adequate. There are no pesticides or toxins stored in any food storage area or preparation area. Appliances in the kitchen were clean and functional. First aid kit was observed to be complete, including a thermometer and a current version of a first aid manual.

BEDROOMS/BATHROOMS: The facility had three (3) bedrooms. Bedrooms #1 and #3 had direct exits to the outside. Bedroom #1’s exit door did not smoothly open and would require residents to have strength and mobility to utilize. Bedroom #3 was not large enough to allow for easy passage between the beds and furniture with a wheelchair or walker, in addition to the second bed blocking the emergency exit. Resident rooms were set up with beds, nightstands, and closet space. The rooms did not contain chairs or provide sufficient clothing storage including chest of drawers. The beds were furnished with box springs, comfortable mattresses and clean linen, which includes, a mattress pad, top and bottom linens, pillowcases, and blanket (if needed).

Beds were observed to be hospital beds each with half rails. Lighting in the rooms appeared adequate. In addition, no bedroom was used as a passageway to another room, bath or toilet. All rooms were free of odors. All window screens were clean and maintained in good repair. There were three (3) bathrooms: two (2) shared and one (1) private. Non-slip mats and grab bars were observed in each bathroom. The hot water temperature was tested and measured within the required range of 105*F to 120*F.

The following corrections are needed prior to licensure:
- Obtain additional activities for residents
- Bedroom #1’s exit door will open smoothly
- Bedrooms need chest of drawers
- Bedroom #3’s exit should not be obstructed
- The Applicant will provide a plan to ensure requirements are met if two (2) residents reside in Bedroom #3
- Remove bed rails

The Applicant is to notify the LPA once corrections have been made, but no longer than 07/07/2026.

Exit interview conducted. A copy of the report was reviewed and provided. This report will be sent to the Centralized Application Bureau (CAB). You will be notified by the CAB Analyst when your license has been approved. You are not allowed to begin operating until you have been notified that your license has been approved by the CAB Analyst. Failure to comply could affect the approval of your license.
