On 06/23/2023 at 11:45 a.m. Licensing Program Analyst (LPA) Doris Whitmore conducted an unannounced visit for the purpose of delivering findings for a complaint investigation regarding the allegation above. LPA met with Heather Strauss and observed 0 children and 3 staff at the time of the visit.

On 04/05/2023, LPA Whitmore initiated the complaint investigation and met with the Early Learning Supervisor, Heather Strauss. LPA toured the facility indoors and outdoors. At the time of the investigation there were no children or staff present. LPA obtained the Personnel Report and Roster.

On 05/12/2023 at 1:25p.m. Licensing Program Analyst (LPA) Doris Whitmore conducted an unannounced complaint investigation and met with Center Director, Sianush Andakyan. LPA explained the purpose of the visit is to continue with the investigation review staff files and conduct interviews. LPA toured the facility indoors and outdoors and observed 40 children and 10teachers. LPA interviewed staff and reviewed staff files electronically.

The Department conducted a full investigation, which included interviews with relevant parties, as well as record review, including documentation as related to the allegation. With the information obtained and interviews conducted the investigation did not provide sufficient evidence to substantiate the allegation. There is no uncleared adult providing care and supervision supervised. Based upon interviews and documents reviewed it did not appear that there are uncleared adults providing care and supervision. Also that the facility is falsifying personnel records.No deficiency cited. Copy of report and Notice of Site visit issued.
