On 10/10/2024 Licensing Program Analysts (LPA) Laticia Thompson and Doris Whitmore met with Siranush Andakyan, Director. The purpose of the visit was an unannounced pre-licensing inspection at the facility to ensure that health, safety and personal rights as required by Title 22 and Health and Safety Regulations governing California Child Care Centers will be met. There were 54 children present during this inspection. This is an application for a total capacity of 98 children, 18 Toddlers ages 18-36 months and 80 Preschooler ages 2- entry into the 1st grade. The child care center will operate Monday – Friday 7:00 a.m. – 6:00 p.m. The fire clearance was approved on 9/9/2024. LPA's toured and measured the on-limits areas. The facility has a total of 6 classrooms. Classroom 1-4 will be dedicated to the preschoolers and classrooms 5 and 6 will be dedicated to the toddlers. Preschoolers will have access to 9 sinks and 9 toilets. Toddlers will have access to 4 sinks, 4 toilets and 2 changing tables. Changing table were not in arms reach of a sink.

The indoor total measurements contain a total of 1,479.43 which will accommodate Applicant’s request for 18 toddlers.

Toddler Outdoor Activity Space measurements are:

The outdoor activity space total measurements contain a total of 2,625.17 which will accommodate Applicant’s request for 18 toddlers.

Preschool Indoor Activity Space measurements are:

The Indoor total measurements contain a total of 3,360.40, which will accommodate Applicant’s request for 80 children.

Preschool Outdoor Activity Space measurements are:

The outdoor activity space total measurements contain a total of 8,932.19 which will accommodate Applicant’s request for 80 children.

Indoor/Children’s Area:

The following was observed.

Parents sign in and out manually and electronically under the Child Plus application. Providers are to ensure that such records are immediately printable upon request for inspection and audit by program staff. For best practice purposes, LPA advised facility there should be a plan in place on how the records would be backed up, and how they would be made available in the event of a power failure or disaster. LPA reference director to review PIN 18-03.1-CCP.



Staff restrooms are located in the front office, rooms 1, 3, 5, and 6. Each classroom has filtered water dispensers for outdoor and indoor activities, operable carbon monoxide detectors, central air, and first aid kits with the required materials (hand held digital thermometers are located in the front office).

The classrooms were clean in good repair. Furniture and equipment were observed to be maintained in good condition, free of sharp, loose or pointed parts. Tables and chairs were observed to meet the needs of the children. There are cots for napping. Windows were in good repair free of chipping paint, dirt, insects or debris. Classrooms have adequate lighting. All play equipment and materials used by children were observed to be age-appropriate. There are cubbies in all classrooms for storage of children clothing and personal belongings. The trash cans located in the bathroom did not have tight fitting lids. The facility is equipped with landline telephones service located in the office. There is cameras to monitor the entrance and rear of the facility. Medication is stored in backpacks with locks that require a key. Medication that requires refrigeration is stored in an area that is inaccessible to children. Ill children will use the front office and staff restroom for isolation The isolation area shall be equipped with a mat, cot, couch or bed.

LPA observed fire extinguishers with an inspection date of 08/02/2024. The required postings are posted were parents and visitors to review. Each licensee shall have a disaster and mass casualty plan of action.

The program provides breakfast, lunch and snacks. The kitchen and food-preparation storage areas were observed to be clean and free of litter and rubbish. Menus are in writing and are posted in advance in an area accessible for review by the child's authorized representative. Copies of the menus as served shall be dated and kept on file for at least 30 days. Menus shall be made available for review by the child's authorized representative and the Department upon request.

LPA did not observe any bodies of water on the premises. There are no firearms on the premises.

LPA’s observed disinfectants, cleaning solutions, and other items that are dangerous stored on a shelf inaccessible to children. Director was advised to store items in storage cabinet that is less inaccessible to children.

Smoking is prohibited on the premises of a child care center as specified in Health and Safety Code Section 1596.795(b).

LPA advised applicant of the following regarding the toddler Program:

101216.4(a)(1) Preschool Program with Toddler Component: A child who is between 18 months and 36 months of age may participate in the toddler program with written permission from the child's authorized representative. No child in the toddler program shall be placed in the preschool program before the age of 30 months without written permission from the child's authorized representative.

Outdoor

The entire outdoor activity grounds are enclosed by a fence to protect children and to keep them in the outdoor activity area. The fence is at least four feet high. The surface of the outdoor activity space has rubber turf and grass for cushioning to prevent injury from falls. The outdoor activity space was observed to be maintained in a safe condition and is free of hazards. There is adequate shade in the outdoor activity area. LPA’s observed benches for resting.

The following items were discussed:

The director was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

LPA reviewed with facility representative the LIC 311A, Records To Be Maintained At The Facility, for child’s records, personnel records, administrative records, and documents to be posted.

Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test.

For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP).

LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of “medication and equipment/supplies, and reviewed children’s, personnel, and
administrative records.

LPA discussed the safe sleep regulations with the director and discussed the Child Care Licensing Safe Sleep webpage at: https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep, as an additional resource. LPA also informed the director of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at: https://www.cpsc.gov/, and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.


The director was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platforms. The following corrections are needed before a licensure will be considered.
  • Replace trash cans that do not have tight fitting lids.
  • Adjust/relocate changing tables within arms reach of a sink.
  • Recommended to relocate cleaning solutions located on shelves
  • Provide Disaster and Mass Casualty Plan

Corrections are due by 11/10/2024.

An exit interview was conducted with plan of corrected items discussed. LPA provided the director with appeal rights and a copy of this report. Final license determination will be made upon review by the Licensing Program Manager and corrections.
