25 | On 01/12/2026, Licensing Program Analyst (LPA) Suzette Ornelas conducted an unannounced Case Management – Incident inspection related to a self-reported Unusual Incident Report (UIR) submitted on 12/18/2025. Upon arrival, LPA met with Director, Siranush Andakyan, and explained the purpose of the visit. LPA observed 72 children in care being supervised by 14 staff.
According to the UIR, on 01/17/2025, child 1 (C1) fell off play structure, hit their chin and obtained a cut. C1 was picked up by their parent, taken to the Doctor, and received stiches.
On 12/18/2025, the UIR (LIC624) was submitted in writing to the department via email.
During today’s inspection, LPA made observations, conducted interviews and obtained pictures and copies of documents. LPA observed the outdoor play structure C1 tripped and fell on to be age appropriate. The play structure label read 2y-5y. Per interview conducted with C1, they were playing and fell and blood came out of their chin. According to Staff 1 (S1) interviewed today, C1 accidentally fell and hurt themselves on the play structure step, there were no other children around. S1 was able to assist C1 by cleaning cut and applying band aid while Staff 2 (S2) contacted parent. Parents took C1 to the doctor and was cleared to return to school the next day.
At this time, no further investigation is needed.
An exit interview was conducted with Director, Siranush Andakyan. A copy of this report (LIC 809) and Notice of Site Visit were provided and must remain posted for 30 days.
