On 10/03/2024, Licensing Program Analyst (LPA) Justeene Tamayo met with Director, Jeana Courson, who granted access to the facility. The purpose of the inspection was to conduct an unannounced follow up case management inspection for a UIR received at Palmdale RO on 09/13/2024. LPA disclosed the purpose of the inspection to the facility representative. When LPA arrived at the facility there were 17 preschool age children in care, with 2 teachers.

Description of Incident: On 09/13/2024, child #1 shoved child #2 to the ground near the bathroom area. Child #2 fell on the floor and split their lip. Child #2 was taken to the hospital and received stitches on their lip.

During this inspection, LPA Tamayo interviewed child #2, child #4 (who witnessed the incident), and Teacher #2 (teacher who applied first aid to child #2). From interviews conducted with staff, and children involved, it was revealed that Teacher #1 had initially advised for child #2 to switch chairs with Teacher #1 so Teacher #1 can have a better view of supervising the day care children going to the bathroom. Child #1 then pushed child #2 because child #2 was in the proximity of child #1. Ice was promptly applied to child #2 lip by Teacher #2, and the parents were immediately notified. Parent of child #2 took the child to the hospital, where child #2 received stitches on their lip. Child #1 was placed on a behavioral contract to ensure this incident does not reoccur. Per Director, child #1 no longer attends the facility due to child #1 violated their behavioral contract. Teacher #1 witnessed the incident, but did not have enough time to react due to child #2 was pushed quickly by child #1, and staff took appropriate measures to ensure child #2 obtained the necessary first aid.

Based on the information obtained, there were no violations of Title 22 regulations, and no deficiencies will be cited at this time. An exit interview was conducted, and a copy of this report was read and provided to the Director, along with a copy of her appeal rights and Notice of Site Visit.
