On June 8, 2026, at 12:46 P.M., Licensing Program Analyst (LPA) Joselito L. Del Mundo met with the facility Director, Aimee Ward to conduct an unannounced Annual/Random inspection for the School Age license in accordance with the facility sketch. A tour of the facility was conducted. LPA observed a total of sixteen school age children inside the classroom with 2 staff providing care and supervision. The hours of operation are 6:30 A.M. - 6:00 P.M., Monday - Friday. The center also has an Infant Program (197409681) and a Preschool Program (197401085). Incidental Medical Services (IMS) was discussed. Licensing fees amounting to $484.00 are due on June 22, 2026.
Indoor/Children’s Area:
Child care center is clean, safe, sanitary and in good repair; all outdoor and indoor passageways, ramps, and other areas of potential hazard are kept free of obstruction; floors of all rooms have a surface that is safe and clean, cleaning compounds are inaccessible and are stored inside the cabinet with a safety lock, furniture/equipment is in good condition; tables/chairs provided to meet children’s needs; all play equipment and materials used by children are age-appropriate. Per Staff 1, the classroom, furniture/equipment, and toys are sanitized at the start of the day, after every activity, after every meal, and at the end of the day. Per Staff 1, they were using the three steps: soap, water, and disinfectant. LPA observed cleaning compounds are stored inside the cabinets. Recalled and or prohibited toys or sleep/play equipment were not observed on the premises. Each child has an individual or portable storage space (cubby, individually labelled with name) for his/her clothing, personal belongings, and or bedding (stored separately). There is a working telephone. There are no window cords accessible to children.
Trash bins have tight fitting lids. The facility has a water fountain inside the classroom. All materials and surfaces are toxic free and are inaccessible. There is no fireplace in the facility. The center has an internal

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

pull-down fire alarm system located near the exit doors. The facility has 2-A:10-B:C type of fire extinguishers. Carbon monoxide devices are in operable condition (checked at 1:27 P.M.)

The facility has two restrooms for boys and girls. LPA observed two toilets in each restroom, paper towels, tissue, and liquid soap. The school age classroom also has three sinks located near the restrooms. LPA also observed trash bins with covers. Water temperature is appropriate. There is an isolation area for children who become ill while in care and it is the main office. The facility always maintains a comfortable temperature. The facility has emergency kit bags inside the classroom and contain emergency supplies. Daily sign in/out sheets and electronic sign in/out using the kiosk are available and completed daily. Smoking is prohibited on premises, no prohibited child care items observed. Firearms/weapons are not allowed or stored on premises. There is no body of water on the premises. LPA did not observe body of water in the premises.


Napping: LPA did not observe sleeping cots. Per Director Ward, school age children do not nap in the facility.
Outdoor: The facility has a playground area for school-age children. Pre-K 2 classroom shares the same playground with the school age children at a different time schedule. Outdoor play equipment was inspected for health, safety, good repair, and age appropriateness. The area was observed to be free of debris, free from hazard, holes, broken items, debris. LPA observed different kinds of play equipment at the outdoor play area and are age appropriate. No bodies of water were observed. According to the director, the facility provides water using the water dispenser and cups. LPA also observed a water fountain inside the classroom.

Teacher child ratios were observed, and staff name recorded. Care and supervision evaluated to determine if the basic needs of children are met and appropriate.

Health Related Services: According to Director, they were currently not providing prescription medications.


Children are inspected for illnesses (wellness policy) as they arrive. Children were physically observed when they arrived at the facility.

Food Service: There is a clean microwave and stove available in the facility. LPA observed the cleaning solutions are stored separately inside the kitchen cabinets. Per Director Ward, the facility provides breakfast, lunch, and snacks when the school is out and provides only afternoon snacks when the children attend school. Meals and snacks are prepared in the kitchen of the facility.

Per Director, transportation is provided for school-age children during field trips. The director also stated that on Wednesday, June 10, 2026, the children will be having their first field trip.

Staff/Personnel Records/Children’s Records: Director (qualified) qualifications were verified, Designation of Responsibility observed, immunization's, TB clearance for staff, health screening, criminal record statement, statement acknowledging suspected child abuse and mandated reporter, and personnel record were observed in file. LPA reviewed the staff files. Based on records review, the staff have current pediatric CPR/First Aid and mandated report training.


LPA reviewed ten children's files, and the records are complete. Per Director Ward, four out of ten children that do not have Physicians’ Reports are enrolled in a public school. The rest of the children that were reviewed started at the facility when they were still little.
Facility Records: Roster, fire/earthquake drills log were conducted last May 12 and 28, 2026, at 10:00 A.M. Per Director, there is a designated Safety Captain in the Center. The Safety Captain organizes and conducts the fire/earthquake drills with children. The drills also include missing and intruder drills.
Posting Requirements: Failure to comply with posting requirements shall result in an immediate civil penalty. The following were observed posted as required: facility license, Personal Rights (LIC613A), Parent’s Rights Poster (PUB 394), emergency disaster plan, earthquake preparedness checklist, classroom schedule, weekly lesson plan, and newsletter.
Documents Provided and or Discussed: Forms and records to keep at the facility and IMS.
Advisory/Other: Emergency bags were observed with supplies readily available.
Sign in and out sheets were reviewed. An electronic sign in/out kiosk is used and is readily available. The parent board was reviewed and has all the required forms posted. Fire/earthquake drills are current.

The Director was advised of the requirement to report Unusual Incidents. Licensee informed to utilize the Unusual Incident Report/Injury Report LIC624B when submitting the report to the department (email address on the website: www.unusualincidentreport@dss.ca.gov). A report shall be made to the department by telephone or fax during the department's normal business hours before the close of the next working day following the occurrence during the operation of family day care home. In addition, a written report shall be submitted to the department within seven days following the occurrence of any events specified above.



The Director was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test as specified in Health and Safety Code section 1597.16.

For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP.).

LPA verified that the lead testing was completed in accordance to the Written Directives (Interim Licensing Standards) outlined in PIN 21-21.1-CCP.

LPA discussed the safe sleep regulations with the director and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed Assistant Director Rodriguez of the importance of checking for and removing any recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Director was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.
