Children and staff disclosed that during nap time, a sheet is placed on every child's cots and there are additional clean sheets in case of an emergency. The children's parents take the sheets and blankets home on Fridays to wash them and return on Mondays.

Staff and children disclosed not observing an infant in the preschool program. Staff disclosed the cabinets containing cleaning materials are always locked and the key is placed on top of the cabinet inaccessible to the children in care. Children disclosed only the teachers have accessed to the cabinets.

Concerning the supervision on the play yard, staff disclosed the various protocols for supervision during outdoor play which include, face to name head count, standing on different ends of the playground to ensure proper supervision. While supervising children the communication among staff is only to address needs or concerns of the children. Children disclosed that during outdoor area children are always supervised by staff.
Concerning the staff supervision of the children while they need to use the bathroom, it was disclosed that staff stand at the bathroom door and supervise the children in the bathroom and the classroom. LPA and LPM observed the restroom setting at the facility and determined that there is adequate supervision. When staff supervise children in the bathroom and in the classroom, the restrooms proximity to the classroom allows for visual supervision of children in the bathroom and in the classroom.

Based on the evidence obtained, the allegations are deemed to be unsubstantiated.

An exit interview was conducted, a copy of this report was provided along with the appeal rights.
