On Thursday, March 21, 2024, at 3:05 p.m., Licensing Program Analyst (LPA), Mayra Rivera conducted an unannounced complaint inspection and delivered findings regarding staff did not meet day care children's dietary needs. Upon arrival, LPA toured the facility and observed 21 preschool children with 2 teachers providing care and supervision.

Based on confidential interviews conducted, and documentation reviewed did not support nor confirm the above allegation. This agency has investigated the complaint alleging staff did not meet day care children's dietary needs. At this time, it is determined that although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation(s) did or did not occur, therefore at this time the above allegation is unsubstantiated. No deficiency given at this time.

Exit interview was conducted with Director Maria Goldman. The licensee was provided a copy of their appeal rights (LIC 9058) and their signature on this form acknowledges receipt of these forms.

On Thursday, March 21, 2024, at 3:05 p.m., Licensing Program Analyst (LPA), Mayra Rivera conducted an unannounced complaint inspection and delivered findings regarding staff did not isolate sick day care child from other children in care. Upon arrival, LPA toured the facility and observed 21 preschool children with 2 teachers providing care and supervision.

During the course of this investigation, Licensing Program Analyst Mayra Rivera conducted confidential interviews. During the interviews it was disclosed when arriving at the facility to pick up their children after receiving a call about their child being sick, their child was not isolated and remained in the classroom.

The above allegation is found to be substantiated. A finding of substantiated means that the allegation has been found to be valid because the preponderance of the evidence standard has been met. Based on information obtained during this investigation, the following Type B deficiency listed on the attached LIC 809D is being cited in accordance with California Code of Regulations Title 22. Upon receipt of this report, the Licensee shall post the Notice of Site Visit and any Licensing report documenting a type “B” deficiency. The report and the Notice of Site Visit (LIC 9213) shall be posted for 30 consecutive days. Failure to maintain posting as required will result in an immediate $100 civil penalty.
