On 11/17/2025, Licensing Program Analyst (LPA) Ali met with director today for the purpose of conducting a permanent relocation inspection. The director is requesting a capacity of 60 preschool-age (PS) children ages 2 – K years. The center was located on a temporary college campus location (2/6/25) and has relocated back to original campus location (8/18/1995). The facility address is 3041 West Avenue K, Lancaster, CA 93536. During this inspection, LPA observed 5 classrooms (CDC 109, 102, 106, 108, 112), 3 preschool bathrooms, and the outside play area designated for each classroom. The hours of operation will be: Monday through Friday from 8:00am-11:30am, 12:30pm-4:00pm. This center is located on the Antelope Valley College Campus in the Child Development Center Building. LPA Ali toured the facility. Fire safety inspection request was approved and received 10/30/25. There are no weapons and no smoking on the premises.

INDOOR ACTIVITY SPACE:
· Furniture and equipment are maintained in good condition, free of sharp, lose or pointed parts. There are a variety of age-appropriate washable toys and equipment, toys, and materials in good condition and in sufficient quantity to allow children present to fully participate in planned activities.
· Disinfectants, cleaning solutions, poisons and other items that could pose a danger if readily available to children were stored and inaccessible to children (stored in the classroom off limits to children with key locks above the sink).
· The isolation is located in the front office for any child who becomes ill during the day.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

The following rooms 109, 112, 108, 106, 102 were all observed and incompliance with Title 22. LPA observed chemicals in top cabinet locked inaccessible to children above the sink in each classroom. Each classroom has a patio that exits to the play yard (fenced) where there is a large play equipment anchored to the ground and tricycle track, and other toys. The childcare center was toured and found to be clean, safe, sanitary, and in good repair to ensure the safety and well-being of children, employees and visitors. Floors of all rooms have a surface that is safe and clean. Tables and chairs were present to meet the needs of the children. Drinking water is readily available, water dispensers. LPA observed operable carbon monoxide detectors and smoke detectors (hard wired). Fire extinguishers are located throughout the facility and fully charged in the green. The center has a working telephone. There are fully stocked first-aid kit in each classroom inaccessible to children. Staff shall conduct a wellness check to ensure that children with obvious symptoms of illness including, but not limited to, fever or vomiting, are not accepted. There is a Sign-in and out procedure in place in front of each classroom door.
SUPERVISION:
Director shall ensure no child is left without the supervision of a teacher at any time, supervision shall include visual observation.
FOOD SERVICES:
Snack Menu must be posted for parents’ review. Full day children bring their own lunch.
Trash cans, including moveable bins, shall have a tight fitting cover that is kept on; shall be in good repair; and shall be leakproof and rodent-proof. LPA observed all trash cans to have tight fitted lid. Kitchen is off-limits by locked door inaccessible to children. Snacks are prepared in kitchen and transported to each classroom by staff. Soaps, detergents, cleaning compounds or similar substances were stored in areas separate from food supplies.
CHILDREN’S RECORDS:
LPA reviewed with Director the LIC 125, records to be maintained at the facility, for child’s records, personnel records, administrative records, and documents to be posted.
TRANSPORTATION:
No transportation at this time.

NAPPING:
Cots used for napping were maintained in a safe condition. Each cot shall be equipped with a sheet to cover the cot or mat and, depending on the weather, a sheet and/or blanket to cover the child. LPA informed Director cots or mats should be wiped with a detergent/disinfectant weekly or when soiled or wet. Bedding shall be individually stored so that each child's bedding is identifiable and no child's used bedding comes into contact with other bedding. Children take their blankets home weekly to wash.
OUTDOOR:
LPA observed a shaded rest area for the children (covered patios from each classroom). Children have safe access to the play yard which is enclosed by a fence to protect children and to keep them in the outdoor activity area. The fence shall be at least six feet high. LPA observed no bodies of water present. The play yard was free of hazards including, but not limited to, holes, broken glass and other debris, and dry grasses that pose a fire hazard. Outside Drinking Water - Staff will bring out water bottles for children.
RESTROOMS:
There is 1 children bathroom located in each classroom. CDC 108 & 112 share a bathroom. There are a total of three preschool bathrooms and three staff bathroom. All toilets and hand washing facilities were maintained. safe, and sanitary operating condition.

The facility does meet Title 22 Regulations. Licensee relocation is approved. Exit interview conducted and report was reviewed with the Director, Angelica Alvarez.

CRIMINAL RECORD REVIEW:
LPA discussed the importance of registering with Guardian to associate and disassociate staff/teachers as needed. The director is reminded of the requirement to report and unusual incidents and/or injuries to the parent/guardian and Licensing within the time frame specified by the regulation and on the form LIC 624B.
HEALTH RELATED SERVICES:
Medications shall be kept in a safe place inaccessible to children. Director has implemented a written plan to record the administration of prescription and nonprescription medications and to inform the child's authorized representative daily when such medications have been given.

LEAD TESTING
Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1- CCP). Director states lead water testing was completed two years ago.
Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02- CCP. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department.
