There are NO weapons, firearms in the facility. At 10:00 am Licensee Evans guide LPA Bailey to the backyard area. LPA Bailey observed in ground swimming pool at 10:04 am ( in the off limit area). Licensee Evans unlock the gate to enter the second backyard where the pool is located. Once in the gated area LPA Bailey observed a six foot fence chain link around the pool with a pad lock that requires a key. Licensee Evans stated this area is off- limits to children in care. At 10:20 am LPA Bailey toured the back yard observed that all areas are adequately fenced and was inspected for hazards. LPA Bailey observed age appropriate toys and games.
At 10:40 am Licensee Evans guide LPA Bailey back into the facility. At 10:47am LPA Bailey tour the children bathroom. LPA Bailey observed children safety latches on the bathroom cabinet. LPA Bailey did not observe any hazardous items in the bathroom.
At 11:18 AM LPA Bailey reviewed the Disaster Plan; Emergency Disaster Drill were reviewed. LPA Bailey observed Licensee conducted Fire & earthquake drill on 03/08/2022. A current Children roster was viewed and maintained for a period of 3 years, even after children no longer are attending the facility.
Licensee has mandated reporter training expired 01/20/2022. LPA Bailey advised Licensee Evans to updated mandated reporter Licensee PED F/A & CPR EXP. 06/25/2023. LPA Bailey reviewed the assistant personal file and observed the file was not in compliance..
Licensee Evans was advised that regulation prohibits the smoking of tobacco in a private residence licensed as a family childcare home during the hours of operation. LPA Bailey reviewed with Licensee Evans different item prohibited in FCCH, no baby bouncers, no infant walkers, No Johnny jumpers, no saucer chairs, no trampolines and any other item that falls into that category are not permitted in the facility.
Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Family Child Care Homes Section 102417. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. Infant Needs and Services Plan: The written infant needs and services plan shall be updated at least quarterly, or as often as necessary to assure its accuracy. Infant Care: LPA advised the applicant to sleep infants where they can be directly supervised at all time. LPA advised against sleeping infants in a separate room. LPA reviewed SIDs, Never Shake A Baby, and safe sleeping practices. Infants should sleep mouth up, on their backs, free of clutter surrounding their sleeping space. Safe sleep concepts were provided. LPA discussed and provided a copy of PIN 20-24-CCP.
The following was discussed with Licensee Evans Individuals who are 18 years of age or older living in the home must be fingerprint cleared prior to licensure. Individuals within one month of their 18th birthday must be fingerprinted immediately. The existing, immediate $100 per individual Civil Penalty has been increased to an immediate $100 per day Civil Penalty, for a maximum of five days for the first violation and a maximum of 30 days for subsequent violations. If an individual has a clearance with the Department a criminal record clearance may be transferred. LIC 9182 Criminal Background Clearance Transfer Request may be used.
The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.
Upon receipt of this report, the Licensee shall post the Notice of Site visit and any licensing report documenting a type “A” deficiency. The report and the Notice of Site visit shall be posted for 30 consecutive days. Failure to maintain posting as required, will result in an immediate $100 civil penalty. A copy of this report shall be provided to the parent/guardian of children currently enrolled by the next business day or immediately upon return. A copy of this report shall also be provided to the parent/guardian of any newly enrolled children for the next 12 months (1 year). The Acknowledgement form must be maintained in each child’s file immediately upon receipt from parent. Licensee was provided with a copy of the parent Acknowledgement of Receipt of Licensing Reports Form during this visit. A copy of the Parent Notification Requirements was also provided to the licensee.
