On November 30, 2022, Licensing Program Analyst (LPA) Monique Ayala conducted an unannounced complaint investigation. A COVID-19 risk assessment was conducted prior to entering the facility. LPA was greeted by office manager Mr. Ochoa who guided LPA on a tour of the facility. Principal, Emily Hooks arrived shortly after. LPA observed 48 preschool children in care with 12 staff members.

Staff not providing adequate supervision to children in care: Complainant alleged, on 11/03/2022 lead teacher Staff #1 (S1) left the room for a break, leaving Staff #2 (S2) and Staff #3 (S3) who are aides alone in the classroom with children.

During the investigation LPA interviewed S2 who stated that S1 looked for coverage of a fully qualified teacher but was unable to find any at the moment. S1 stepped out to tend to a personal need leaving S2 and S3 alone with children for less than 1 minute when another fully qualified teacher Staff #4 (S4) arrived

in the classroom to provide care and supervision. Based on interviews conducted, the above allegation is deemed SUBSTANTIATED. S2 and S3 are not fully qualified teachers and were left in the classroom for a short period of time to provide care and supervision to children.

A Type B deficiency is being cited under Title 22 Regulation code, 101229(a)(1), see LIC9099D.

An exit interview was conducted and a copy of this report was provided to principal, Emily Hooks along with a Notice of Site Visit. Appeal Rights were provided and explained to principal.
