Licensing Program Analysts (LPA's) Roxana Lopez and Jessica Hopkins-Hernandez conducted an unannounced Case Management inspection due to 5 incidents that were reported to the Department. A COVID 19 risk assessment was conducted. LPAs met with Principal Emily Hooks who guided LPAs on a tour of the facility. Census was taken.

On September 11th, 2023, an incident was self reported to the Department via Email by the facility who reported a staff alleges a child's personal rights were violated



On October 4th, 2023, an incident was self reported to the Department via Email by the facility who reported a parent alleges their child was inappropriately touched by another child.

On January 5th, 2024, an incident was self reported to the Department via Email by the facility who reported a child entered a classroom with another child and that child's parent.

On January 17th, 2024 two incidents were self reported to the Department via Email by the facility who reported that a new student had walked out of the classroom when another child was being picked up and that two children were left unattended in the bathroom during a break transition.

All reports were reported within the required 24 hours. The purpose of the inspection was to obtain additional information regarding the incidents reported to the Department. Regarding incidents that occurred on 9/11/23 and 10/4/23 interviews were conducted, no disclosures were made.

Regarding incident that occurred on 1/5/2024 child was observed to be by a classroom door by a staff member that reminded child to stay outside- however interviews disclosed that child did go inside the classroom with another child and the child's parent with out staff supervision. Child was observed in the classroom by a staff member coming back from break- this was approximately 3 minutes after child was observed by the door.

Regarding incident that occurred on 1/17/2024: Two children left unattended in the bathroom: Interviews disclosed that staff supervising the bathroom announced going on break and left the area before another staff arrived to supervise the area- leaving two children unattended.

The second incident on 1/17/2024: child left the classroom while other children were being picked up. Interviews disclosed that child was observed to be outside by the apparatus by a staff member coming out of the office and by a parent picking up their child. Child was then brought into the classroom. Interviews corroborated that child might have left the classroom during transition from the carpet to the tables and as other parents were picking up. Conflicting statements were made on the time child was unsupervised- from 3-10 minutes.

Interviews conducted disclosed that incidents which occurred 1/5/2024 and both from 1/17/2024 resulted in lack of supervision which is an immediate danger for the health and safety of children in care.

Based on the LPA’s observations and records reviews, the following deficiencies listed on the attached LIC 809D (deficiency page) are being cited in accordance with California Code od Regulations Title 22. Deficiencies that are being cited need to be cleared to protect the children’s health and safety.

Also, LPAs Roxana Lopez and Jessica Hopkins-Hernadez informed the facility representative to provide a copy of this licensing report dated 1/25/2024 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.

A notice of site visit was given and must remain posted for 30 days.
