On 12/16/24 at 8:05 AM, Licensing Program Analyst (LPA) Jeanine Lipsey conducted, a Case management incident inspection. The incident was reported to Community Care Licensing on 12/12/24 via phone. The facility reported the Unusual Incident to the Department within the required 24 hours of occurrence. The incident which occurred on 12/12/24 was that the child (C1) fell and injured their head against the fence which produced a small cut..

Upon arrival, LPA was guided on a tour of the facility by Staff Katherine Nathan , to whom the purpose of the inspection was announced. 

Census:  There were forty seven children being supervised by thirteen staff.

On todays visit, LPA interviewed 3 staff, took photos of the area in question and measured the climbing apparatus (bridge) the child fell off of, obtained a copy of the facility roster, a copy of the message through "Brightwheel", a copy of the incident recorded on video from a staff members phone and photos of the child's injury. LPA observed the area of where the bridge was located near the fence.

The incident occurred after lunch at about 12 or 12:30pm, while the child was playing on the playground. The bridge was located against the fence. It is 11 inches in height, age appropriate, and deemed safe for the children to play on. The bridge has 2 small steps in the front and 2 steps in the back. C1 walked up the stairs and when C1 came down the stairs, they lost their balance, fell and hit their head on the fence.

The video captured the incident and LPA observed the teacher sitting on the ground near the child within arms length and was able to attend to the child as soon as they fell. Per staff the child was not is distress long and kept playing. As the child continued to play the teacher noticed the hair getting darker and with further examination notice the child had a small cut about 1 inch long. The teachers washed the area and applied a ice pack and the bleeding stopped. The parents were called immediately and within 20 minutes the parent arrived to collect the child. Per staff C1 was seen by a doctor was released to go home. Staff #3 S3 messaged the parent through" Brightwheel" to see how the child was doing after the doctors visit. The parent replied the child was fine and they were going to keep C1 home on Friday 12/13/24.

To prevent a incident from happening again, the following steps will be in place to ensure the safety of the children:
1. The bridge was temporarily removed.
2. After location of the designated area is finalized and discussed with staff, the bridge will be placed on the playground away from a hard surface where a child could be injured.
3. The designated area will be taped on the ground surface so all staff will know the permeant placement.
4. The staff will continue to supervise all children during outside play time.
5. The director will send photo of designated area via email by 12/20/24.

Based upon information received and the interviews conducted, The department finds there was no deficiency in compliance. No additional follow up is necessary regarding the incident reported.

Exit interview conducted and report was reviewed with Director Adena Amalian.  A Notice of Site Visit was given and must remain posted for 30 days.
