On 1/22/25 at 8:17am, Licensing Program Analyst (LPA) Jeanine Lipsey made an unannounced visit for the purpose of delivering findings of a complaint received by the Department associated to Complaint Control Number 58-CC-20241028125618. LPA met with Director Adena Amalian, to whom the reason for the inspection was announced. LPA toured the facility and observed:

RM 1 (Caterpillar): 6 infants with a ratio of 2 teachers
RM 2 (Butterfly): 12 infants with a ratio of 3 teachers
RM 3 (Lady Bug): 7 infants with a ratio of 3 teachers
RM 4 (Dragonfly): 6 Infants with a ratio of 3 teachers

Throughout the course of the investigation, LPA Lipsey made observations of staff actively supervising children, obtained a copy of the LIC 9040 Child Care Facility Roster, photos and interviews from the Director, six staff and six parents.

Pertaining to the allegation, Toxic substances are accessible to children in care. Per reporting party, they have seen bleach in reach of children. During LPA’s inspection, bleach was not seen near the children. LPA observed bleach to be in the locked cabinet high on the shelf. Per staff soap and water is used to clean, and if beach is used, it is before the children are present. Per the parents interviewed there were no disclosures of safety concerns.

Pertaining to the allegation Staff did not provide adequate supervision to children in care. Per reporting party, There is lack of supervision, teachers are always on their phones. During LPA’s inspection, Staff were observed actively supervising the children and cell phones were not seen being used. When asked for what reason would they have to be on the phone, staff stated the phone is used to document events that go on with the children. Staff have the Brightwheel App on the phone. This App is used to document sleep logs, diaper changes, ouch reports, feedings and they take pictures of the children to send to the parents. LPA observed Brightwheel messages to the parents from staff. The director stated the cell phone is used for Brightwheel purposes only and no complaints of personal cell use was known. Per staff they have not seen staff use the cell phones for personal use.

Based upon evidence obtained during this investigation, the allegations Toxic substances are accessible to children in care and Staff did not provide adequate supervision to children in care, have been determined to be Unsubstantiated. A finding that a complaint is Unsubstantiated means that although the allegation may have happened or is valid, there is not a preponderance of evidence to prove that the alleged violation occurred.



Notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with Director Adena Amalian.

The Director will continue exterminator maintence monthly until there is no more activity. Director will send copies of the reports to LPA via email. Due dates are Febuary 2025 and March2025. Monthly Director will send pest control log to LPA that pest checks are being performed til 5/2025.

Based on observation and interviews, the licensee did not comply with the section sited above in that Licensee did not ensure the center was clear of mice, which poses an immediate health, safety or personal risk to children in care.

Throughout the course of the investigation, LPA Lipsey made observations of staff actively supervising children, obtained a copy of the LIC 9040 Child Care Facility Roster, copies of the exterminator reports, photos and interviews from the Director, six staff and six parents.

Pertaining to the allegation, Staff did not ensure facility is kept free of pests. Per reporting party, they see mice, mouse droppings and traps in the classroom. Parents interviewed did not present concerns related to the above-mentioned allegation. Staff interviewed stated they have not seen any mice. Per the director, there were no recent sightings of mice since July 2024. The center was serviced on 11/13/24 and 12/4/24 by the Western Exterminator Company and per their report, no rodent activity was noted. LPA conducted two unannounced inspections, 12/18/24 and 1/9/25, during those inspections, LPA toured the designated infant area and did not observe mice in the infant area. However, LPA took photos and observed mouse traps with droppings behind the refrigerator in the classroom.

Based upon evidence obtained during this investigation, the above allegation has been determined to be Substantiated. A finding that the complaint is substantiated means that the allegation is valid because the preponderance of the evidence standards has been met.

A Type A citation is being issued. See LIC9099-D for deficiency cited.
LPA Jeanine Lipsey informed licensee Director- Adena Amalian that this report dated 1/22/25 documents
One Type A citation which shall be posted for 30 consecutive days as there is immediate risk to the health,
Safety, or personal rights of children in care.

Also, LPA Jeanine Lipsey informed the licensee to provide a copy of this licensing report dated 1/22/25 that documents any Type A citation to parents/guardian of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed
Acknowledgement of Receipt of Licensing Report (LIC9924), or written statement, must be placed in the child's file for verification.
