Licensing Program Analyst (LPA) Silva Garibyan conducted an unannounced Required - 3 year inspection. LPA met with Bernice Zachariash, Director, who guided analyst on a tour of the facility. This is a preschool program serving up to 135 children. The facility consists of 9 classrooms and children are grouped by ages. The facility operates Monday through Thursday, from 8:30 am to 3:45 pm and Friday from 8:30 am to 1:30 pm. . Director's office used as isolation area for sick children. LPA observed 130 children in care being supervised by nine teachers and 17 aides during this inspection. The center has two licensed private armed security guards operating in the school parking lot and at the entry. LPA verified that all adults present in the facility have obtained criminal record clearances and are associated to the facility. There are no bodies of water on the premises during the visit. LPA observed 12 children in Room 1(2 year old) ) supervised by one teacher and two aides, 14 children in Room 2 (3 year olds) supervised by one teacher and one aide, 14 children in Room 3 (2 year olds) supervised by one teacher and two aides, 15 children in Room 4 (3-4 year olds) supervised by one teacher and two aides, 15 children in Room 5 (3 - 4 year old), supervised by one teacher and two aides, 12 children in Room 6 (3-4 year olds) supervised by one teacher and one aide, 14 children in Room 7 (3-4 year olds) supervised by one teacher and two aides, 17 children in Room 8 (Pre-K, 4-5 year olds) supervised by one teacher and two aides, and 16 children in Room 9 (Pre-K, 4-5) supervised by one teacher and one aide. LPA verified that all adults present in the facility have obtained criminal record clearances and are associated to the facility. LPA reviewed the sign in and sign out sheet to verify the census. LPA observed the following postings in each classroom and by Director's office: License, LIC 613A, the PUB 393, Child Care Center Notification of Parents' Rights Poster, LIC 610 Emergency Disaster Plan with the telephone number of the local health department, information on child passenger restraint systems (PUB269). Furniture and equipment were inspected for age appropriateness and good repair. The classrooms were inspected to ensure that the floors have a surface that is safe and clean.

A walk through of the classrooms was conducted, classrooms were found to be clean and free from any potential hazards. Furniture was found to be in good repair and age appropriate.

There is adequate heating, lighting and ventilation. Drinking water is readily available in each classroom. Drinking water is available inside the classrooms in the form of water dispensers and disposable cups. Pitchers and disposable cups are used for outdoor drinking water on the yard. There are first aid supplies in each classroom and school Principle's office. .

Napping equipment and bedding was inspected for good condition, appropriate storage and cleanliness. LPA observed cots available for each child are being sanitized once a week. Bedding is stored separately. Napping children are properly supervised within ratios. Isolation area for sick students is located in the Director's office. LPA observed adequate arrangements for isolation and care of ill children.



LPA inspected and observed 2 clean bathrooms. There are a total of 9 toilets and 6 sinks. Toilets and sinks are functioning properly and are age appropriate. LPA observed soap, toilet paper and paper towels readily available. Water temperature was tested and is appropriate. Bathrooms were found to be clean. There is adequate lighting/ventilation in the bathroom areas.
Children are inspected for illnesses as they arrive. A review of medication policy indicated that Medication is administered by Director, who documents the date, time and dosage onto a log. Medications are stored securely and inaccessible to children in the Director's office. Page 2

The facility provides snacks only. Children bring their own lunches. A hot lunch program is offered and is prepared in an off-limits kitchen on site on the second floor. The second floor of the facility is made inaccessible by gates at the bottom of the staircases that contain latches on the inside of the gates to prevent children from opening them. Menus were observed posted on the parent board. The food preparation area is upstairs in the kitchen and meals are taken into classrooms. Sharp utensils are stored in the upstairs kitchen or high on a shelf in the classroom, inaccessible to children. Opened snack items are properly stored in air tight containers inside the classrooms.

Outdoor equipment was inspected for safety, cushioning material, good repair and age appropriateness. Required shade, drinking water and fencing were inspected. Fencing around the perimeter of the play area is at least 4 feet high. Play area was inspected for hazards and inaccessibility to bodies of water. The facility is equipped with a working smoke detectors and Carbon Monoxide detectors. LPA observed five fire extinguishers (2A 10BC) outside the classrooms with service date of 12/15/2023. First Aid Kit was observed to be complete. Fire Drill log was observed to be complete and current. The facility has sufficient cots for napping. LPA observed changing tables in Room 1, Room 2, and Room 3. LPA reviewed a sample of children’s files and observed files were complete. LPA reviewed a sample of staff files and observed files were complete. CPR, Pediatric First Aid (Director completed on 08/24/24) and Mandated Reporter Training (Director Completed on 12/09/2022) were observed to be current. The name of the child care center director or fully qualified teacher designated to act in the director’s absence has been reported to the Department.

Director was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.
Director was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.
Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for
drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test.
For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP).
LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP.

A notice of site visit was given and must remain posted for 30 days.
