Licensing Program Analyst (LPA) Silva Garibyan conducted an unannounced Required - 3 Year inspection on this date. LPA met with Georgina Garcia-Figueroa, Director, LPA explained the purpose of the inspection and was guided on tour of the facility. This is an Infant program serving up to 18 infants in two classrooms. Indoor and outdoor spaces for infants was observed to be physically separate from space used by children under the preschool license (License # 197403353) . The entrance to the facility is locked and parents enter using a doorbell. There were 11 infants in care during visit supervised by five staff ( two infants in babies' room and nine infants in walker's room). Teacher-child ratios are in compliance and staff fingerprints have the required clearances. The facility operates Monday – Friday 7:00 a.m. – 6:00 p.m.

LPA observed the following postings located on a licensing board near the entrance to the facility: License, LIC 613A, the PUB 393, Child Care Center Notification of Parents' Rights Poster, LIC 610 Emergency Disaster Plan with the telephone number of the local health department, information on child passenger restraint systems (PUB269), and a contingency plan for action during fires, floods and earthquakes. Menus were observed to be in writing and posted. Furniture and equipment were inspected for age appropriateness and good repair. Toys were observed to be safe, and do not have sharp points, edges, splinters and are not made of small parts that can pose a choking hazard. LPA did not observe any baby walkers or any items that fall into that category present in the facility. The classrooms were inspected to ensure that the floors have a surface that is safe and clean. LPA observed children have their own cubbies to store their belongings in each classroom. Napping area was observed to be physically separate from the indoor activity space. The facility was observed to be maintained at comfortable temperature. Page 1

Infant nap room was observed with 4 standard size cribs that are in compliance and available for all infants who are unable to climb out of a crib. Cribs were arranged to not limit the ability of staff to see the infant. Crib mattresses were observed to be: Covered with vinyl or similar moisture-resistant material and maintained in good condition. Floor cots were observed available for infants who have the ability to climb out of a crib. LPA advised each crib, cot occupied by only one infant at a time. Each infant's bedding is used for them only. Facility provides the sheets and has a washer and dryer for laundering. Per Director, sheets are washed daily.
LPA observed a variety of age-appropriate equipment, toys and materials in good condition and in sufficient quantity to allow children present to fully participate in planned activities.

LPA observed changing tables in both infant rooms to have a padded surface no less than one-inch thick and be covered with washable vinyl. Sides were raised and measured to be at least three inches high. LPA observed changing tables to be maintained in good repair and safe condition. Changing tables were observed to be within arm's reach of a sink. General sanitation was observed. Cleaning supplies are kept locked and/or inaccessible.

The toilet and hand washing sinks were observed to be maintained in safe and sanitary operating condition. Infant hand washing sinks were observed to deliver cold water. General sanitation was observed. Cleaning supplies are kept locked and/or inaccessible. Children within the isolation area will use staff bathrooms if needed.

Infant outdoor activity space was observed to be physically separate from space used by children in the preschool program. LPA observed a shaded rest area for the children. Outdoor area was observed to be free of hazards including, but not limited to, holes, broken glass and other debris, and dry grasses that pose a fire hazard. Facility does not have sandbox. No bodies of water were observed at facility.

LPA observed the playground to be enclosed by a fence to protect children and to keep them in the outdoor activity area. The fence was observed to be five feet high.

LPA observed tables and chairs used to seat infants during feeding. LPA observed Infants who were unable to sit unassisted being held by a staff person for feeding. No highchairs observed during the visit.

Bottles, dishes and containers of food brought by the infant's authorized representative were observed be labeled with the infant's name and the current date.

Facility has a kitchen and provides all foods. Menu shows facility provides morning snack, lunch, and afternoon snack. Kitchen, food preparation and storage areas were observed to be kept clean and free of litter and rubbish. Menu's were observed in the classrooms and in the kitchen. Kitchen was in good repair. The kitchen is equipped with two refrigerators, freezer, and stove. Snacks are stored in air tight containers in the kitchen. Food was observed to be safe and of the quality and in the quantity necessary to meet the needs of the children. LPA observed all food to be stored, prepared and served in a safe and healthful manner. LPA observed soaps, detergents, cleaning compounds or similar substances to be stored in areas separate from food supplies.

The facility has one or more functioning smoke detectors and carbon monoxide detectors that meet statutory requirements. LPA observed three fire extinguishers (3A40BC) serviced on 08/14/2024. Trash cans were observed with appropriate lids. First aid kit was observed to have: bandages, adhesive tape, scissors, tweezers, and antiseptic solution.

LPA reviewed sign in and sign out sheets to ensure children are being properly signed in and signed out by the authorized representative. LPA conducted a Children's Records Review for completeness and to ensure children have proper forms such as Identification and Emergency forms, Immunization records, Health History, Personal Rights, Parents Rights, Sleep Plan (LIC 9227) and sleep plan on file. The children's roster was available and current. Page 3

Staff Records were reviewed to ensure the following: Criminal record clearance or criminal record exemption, education credits required for Infant Care, current CPR/First Aid and health preventative practices, proof of immunization against pertussis and measles and influenza and/or flu declination, Mandated Reporter training. LPA observed at least one staff member to have current EMSA approved pediatric CPR/1st Aid. Medications not administered at this time.

Director was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

LPA discussed the safe sleep regulations with Director and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee [or facility representative] of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for
drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test.
For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP).
LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP

Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. Director was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

A notice of site visit was given and must remain posted for 30 days.
