Licensing Program Analyst (LPA) Shandra Powell conducted a Case Management Deficiencies Inspection while during a walk-through of the facility and counting the children in care LPA observed that there were 13 children in two classrooms with one Teacher in the 4 and 4/5 age classrooms at the facility. This poses an immediate risk to the health and safety of children in care.

A copy of this report shall be provided to the parent/guardian of children currently enrolled by the next business day or immediately upon return. A copy of this report shall also be provided to the parent/guardian of any newly enrolled children for the next 12 months (1 year). Acknowledgement of Receipt (LIC 9224 form) must be maintained in each child’s file immediately upon receipt from parent. Licensee was provided with a copy of the Acknowledgement of Receipt of Licensing Reports (LIC 9224) Form during this visit.

Exit interview conducted with Director. Appeal rights explained and provided.



"Notice of Site Visit" and report was issued. Notice of Site Visit must remain posted for 30 days. Failure to do so will result in a $100.00 civil penalty.

One classroom was corrected during inspection. An extra staff member arrived and placed the classroom in ratio.
Director will make changes by POC date of 12/31/2022. Declaration will be provided via email and changes also will be noted within the declaration.
