LPA did not observe, nor was information provided via interviews that provided sufficient evidence to substantiate the allegations of facility is operating outside the scope of their license, there was no information obtained that the facility was caring for children outside of the age range specified on the license; the facility is operating out of ratio, on 04/07/2025 LPA Whitmore checked each classroom with the children and staff and the facility was not out of ratio, there was also a review of the sign-in/sign-out sheets. Did not provide adequate supervision to day care children, LPA observed outdoor time staff were spreaded out on the playground. Staff were in designated areas on the playground. LPA did not observe staff in a circle having conversations; staff expose children to toxic chemical odors. LPA went to each classroom and did not observe or smell any toxic chemical odors.
Therefore, the allegations are deemed unsubstantiated. Meaning, although the allegations may have happened or valid, there is not a preponderance of the evidence to prove that the alleged violation(s) did or did not occur, therefore the allegations is unsubstantiated. No deficiencies cited.
An exit interview was conducted, copy of this report was read, appeal rights along with Notice of Site Visit were provided. Notice of Site Visit is required to be posted for 30 days.
