25 | On 07/14/2023, Licensing Program Analyst (LPA) Justeene Tamayo met with Director Loretta Pilafas who guided LPA on a tour of the facility. The purpose of this visit was to conduct a Case Management - Incident Follow Up Inspection for an Unusual Incident that occurred on 06/22/23. The Unusual Incident was self reported within the time frame specified by regulations. Upon arrival, LPA observed 47 preschool children in care with 4 staff present, and the Director.
LPA Tamayo followed up with staff and children involved regarding the incident.
Description of incident: On 06/22/23, Child #1(C1) was laying on the cot. C1 pulled his pants and underwear down and exposed himself. Child #2 (C2) was laying down on a cot next to C1. C2 came over and used their finger to "poke" C1 on the private part. Staff #3 (S3)observed the behavior and immediately stopped the situation and talked to the children about keeping pants and hands to self.
After interviews conducted, it was revealed Staff #3 had witnessed the incident, and took appropriate measures to ensure the health and safety of each child. No deficiencies are being cited at this time.
An exit interview was conducted and a copy of this report was provided to the Director, along with Notice of Site Visit and appeal rights.
