Licensing Program Analyst (LPA) Maddox met with Director, Ms Genesis today for the purpose of conducting an unannounced Required 1 year inspection. There are 3 Preschool Classrooms at this location. This is a combination center with a licensed Infant (197403568) and School age component (197403566). Each component maintains a physical separation indoors and outdoors. Days and Hours of Operation: Mon - Fri from 6:30 AM to 6:30 PM.

Furniture and equipment was inspected for age appropriateness and good repair. Telephone service, heating, lighting and ventilation were evaluated. LPA observed individual cubbies with children’s names labeled for their belongings. Children have access to water through a water fountain. The front office is designated and staff room are designated as isolation areas.
Age appropriate sinks and toilets were inspected for availability and good repair. Toilets flush properly; toilets and sinks are reachable by the children. The restroom has adequate toilet paper and paper towels available. Restroom was found to be clean and sanitary. There is a separate staff bathroom that will also be designated as an isolation bathroom. The staff restroom was free from disinfectants, cleaning solutions, poisons, or any other dangerous items that could pose a health and safety risk to the children in care. Each Preschool classroom has it's own bathroom.

First Aid supplies, smoke detector, carbon monoxide and fire extinguishers were observed and in operable condition. Medications are locked and stored inside of a cabinet in the site supervisor's work space. Trash cans with tight lids were observed. The children are provided with snacks. Food was inspected and it was properly labeled, stored, and within expiration date. Kitchen area is adequately equipped, clean, and free from hazards. The kitchen has a sink, stove, refrigerator, and freezer. The menu and allergy list are both posted in the kitchen and accessible for parent’s review.

The outdoor play area was inspected and observed to be free of hazards, loose, or sharp objects. Equipment was inspected for safety, cushioning material, good repair and age appropriateness. Climbing structures, slides, and play equipment were found to be securely anchored with adequate resilient cushioning material underneath and around the perimeter (rubber matting and wood cips. The outdoor area has a working water fountain accessible to children, although the facility provides them with a water jug and water cups during outdoor activities. The playground is well fenced all around (fencing at least 4ft high) and no bodies of water were observed in the outdoor play area. The playground area has a shaded rest area for the children and meets all of the Title 22 regulations.



Children files were reviewed and found to be complete. The facility roster and fire drills were up to date, and all staff have been fingerprinted and associated to the designated license number. The site supervisor is currently CPR and Pediatric First Aid certified, which expires on exp 12/24.

During the inspection LPA observed adequate teacher child ratio in the classroom. Care and supervision was evaluated and determine basic needs of children are appropriate and are being met. The parent board was reviewed and has all of the required forms posted accessible to parents.

This facility provides Incidental Medical Services – IMS. LPAs reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. Center conducts required Earthquake/Fire Drill, last drill conducted 4/6/2023. Required Lead Testing has been completed, all water fountains (inside and outdoors) have passed, the kitchen had a failed test, however, Director provided verification the kitchen faucet has been repaired and has since passed.


An exit Interview was conducted, a copy of this Report and a Notice of Site Visit was provided to Director, Genesis Benitez.
