****THIS IS AN AMENDMENT OF THE ORIGINAL REPORT CREATED ON 04/04/24. THIS IS TO AMEND THE FINDINGS.

On 5/31/24, Licensing Program Analyst (LPA) Brittanee Cleveland arrived at 10:30 AM to facility stated above to amend findings from the original document for the above stated allegation. LPA met with Thania Garcia, Director, who guided LPA on tour of the facility. There were 17 staff and 96 children at the time of visit.

LPA conducted interviews with RP and staff. After reviewing the evidence from the previous visits, S3 did indicate that their hand was on child’s head. This is a personal rights violation and will be issued as a type-B deficiency.

Based on the interview conducted and new information recieved, the preponderance of evidence standard has been met, therefore the above allegation is found to be Substantiated. California Code of Regulations, Title 22 (code) is being cited on the attached LIC 9099D.

The Notice of Site Visit must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00.

An exit interview was conducted with Thania Garcia, Director.
