On 08/10/2026, Licensing Program Analyst (LPA) Lisa Clayton arrived unannounced at the Wiz Child Center to deliver the finding on a complaint. LPA Clayton was greeted by Administrator Jerry Hale and observed children 24 being supervised by 7 fingerprint cleared staff.

On 05/12/2026, El Segundo Child Care Regional Office received a complaint alleging the following:
• PERSONAL RIGHTS: Staff threaten children in care.
• PERSONAL RIGHTS: Staff yell at children in care.
• PERSONAL RIGHTS: Staff had inappropriate interaction with daycare child.

On 05/18/2026 LPA Clayton conducted the 10-day visit, toured the CCC inside and outside for a Health and Safety Inspection, interviewed children and staff.
During today’s inspection, LPA Clayton toured the CCC inside and outside for a Health and Safety Inspection, and interviewed staff.

A full investigation was conducted which included observations, records reviews, and interviews. Based on interviews and record reviews, which were conducted and recorded, the preponderance of evidence standard has been met, therefore the above allegations are found to be SUBSTANTIATED. Record reviews and interviews showed that staff threatened a child in care, yelled at a child in care, which caused the inappropriate interaction with a child in care. California Code of Regulations, Title 22 Division 12, Chapter 1, and Article 06, are being cited on the attached LIC. 9099D.

LPA Lisa Clayton informed Administrator Hale that this report dated 08/10/2026 documents 1 (one) Type A citation which shall be posted for 30 consecutive days as there is immediate risk to the health, safety, or personal rights of children in care.

Also, LPA Clayton informed Administrator Hale to provide a copy of this licensing report dated 08/10/2026 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.

The notice of Site Visit (LIC 9213) must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of 100.00.

LPA Clayton offered Admin Hale to participate in the Department’s Technical Support Program (TSP) which is the non-enforcement service of Community Care Licensing to help providers with additional resources/training regarding personal rights and overall operation of a Child Care Center.



Exit interview conducted and report was reviewed with Administrator Hale. LPA Clayton posted a Notice of Site visit which is to remain posted for 30 days.

Admin Hale agreed to conduct an all staff meeting to discuss personal rights for children in care to ensure that children are free from corporal or unusual punishment, infliction of pain, intimidation, threat, or other actions of a punitive nature.

This requirement has not been met as evidenced by: LPA Clayton review of information provided by the RP and staff interviews showed that staff S5 yelled at and threatened a child in care which caused the inappropriate interaction with a child in care, which poses an immediate Health and Safety risk to children in care.

Admin Hale agreed to review the California Preschool Instructional Network (https://cpin.us/) a professional development system that provides high-quality staff training, instructional resources to early childhood educators to develop a plan of correction to be provided to the department no later than 08/20/2026.
