25 | On Friday, January 30, 2026, Licensing Program Analyst (LPA) Hanna Cha met with Licensee Jacklin Avakian. The purpose of the visit was to conduct an unannounced Annual/Random inspection of the facility. LPA disclosed the purpose of the inspection to the licensee. LPA observed licensee providing care and supervision to two infants, two preschool-age children, and one school-age child. The current hours and days of operation are Mondays through Fridays from 7:00 a.m. to 6:00 p.m.
Physical Plant: This is a two-story family. There is a living room, dining room, kitchen, four bedrooms, three restrooms, and attached garage, and back yard. The off-limit areas are the entire second floor with three bedrooms (child safety gate observed), kitchen, dining room, and two restrooms.
Indoor/Children’s Area: LPA inspected the indoor/children’s area for safety, cleanliness and good repair. Main care is provided in the living room and attached garage. Per licensee, the attached garage is only used as a play area. Per licensee, children do not eat or nap in the attached garage. LPA tested the carbon monoxide and smoke alarm to be in operable condition. LPA observed the fire extinguisher, fully green. Per licensee, there are no firearms/weapons on the premises. Per licensee, there is no smoking in the home. Per LPA observation, there is a barricaded fireplace. Per licensee, fireplace is not utilized.
Bathroom: LPA observed the child care bathroom for safe and sanitary conditions. The toilet and faucet are in good operable condition. LPA reminded licensee to keep all hazardous items inaccessible to children.
Kitchen: LPA inspected the kitchen for safety, sanitation, and good repair. LPA reminded Licensee, if food is brought from the child’s home, the container shall be labeled with the child’s name and properly stored or refrigerated. Licensee participates in the food program and provides all meals during operational hours.
