On February 18, 2025 Licensing Program Analyst (LPA) Portia Bowden conducted an unannounced Annual Inspection at the above facility. LPA met with Director Yolanda Carter, explained the reason for visit, provided LIC 125 (Entrance Checklist) and was guided on a tour. At 12:45PM Owner Debra joined inspection. Per Director the Facility operates Monday – Friday 6AM-6PM, and does not provide transportation. Also housed on premises is an Infant Program under facility number 197404745. LPA observed all required documents on the facility licensing boards (PUB 269, PUB 393, LIC 610, LIC 9224, LIC 613A). LPA observed an ER drill log with last Fire drill conducted 4/24.

This is a preschool program serving ages 2 to 5 years old, consisting of 2 classrooms, 1 open nap area and an enclosed outdoor play area. LPA observed 14 children napping supervised by 1 teacher and 1 aide. Per Director there are 15 children enrolled. Capacity and limitations as specified on the license are being maintained. Per facility rep there are no children in care who require Incidental Medical Services. LPA reviewed sign in and out sheets and observed 15 children signed in.

The following was observed during the tour of the facility: furniture and equipment were inspected for age appropriateness and good repair. Telephone service, heating, lighting, and ventilation were evaluated. At 12:30PM LPA observed a wall heater in the napping area properly barricaided. At 2:30PM LPA observed an operable Carbon monoxide detector on the wall of the nap area as well as an inline fire system throughout facility. At 12:30PM LPA observed cots for sleeping as well as thick mats. Per Director Linens are sent home on Friday’s to be washed by parents and returned to facility. LPA observed 2 individual bathrooms for children to use each with 1 toilet, and 1 sink in good repair. Per Director potty training is provided by the facility. General sanitation was observed.

Per PIN 21-21-CCP lead testing to be conducted for all equipment used for drinking consumption. At 12:50 PM LPA observed Lead testing results dated 2/24. Per Director they use water system connected to the building for drinking. Per Director children in care use cups provided by facility to fill up and take outside for drinking water.

At 12:55PM LPA observed disinfectant stored on a high shelf inside the classroom inaccessible to children in care. Per Director there are no poisons on the premises.

Facility provides Breakfast, lunch, PM snack, and is not a participant in the food program. Per Director there is 1 child in care with food allergies. Per Director facility chef is aware of all food allergies and provides substitutions. Per Director outside food is permitted, LPA reminded Director to ensure all outside food brought into the facility is labeled with child’s name and dated.



Outdoor play is conducted in an enclosed playground at 1:00PM LPA observed a two story play apparatus in a safe condition, free of sharp, loose or pointed parts, with adequate cushioning underneath. There is adequate shade in the play yard with age appropriate toys. LPA advised that no children shall be left without the supervision of a teacher at any time. All materials accessible to children were observed to be toxic free. There are no firearms stored on the premises. There are no pools or bodies of water on the premises.

LPA Reviewed 3 children’s files and observed them all to be complete.

LPA reviewed 4 staff files and observed 2 out of 4 to be missing Mandated Reporter certificates. Per Owner Debra she will have staff obtain current certificates and submit photo to LPA via email. A Type B Deficiency will be issued.

The name of the child care center director or fully-qualified teacher(s) designated to act in the director’s absence has been reported to the Department. The person who signs the child in/out of the facility shall use their full legal signature and record the time of day.

Facility representative was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for


drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test.
For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP).
LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP.

Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. 1 Type B deficiency was observed during today’s inspection for 2 out of 4 staff missing Mandated Reporter Certificates.

Exit interview conducted and report was reviewed with Owner Debra. Appeal Rights and a A notice of site visit was given and must remain posted for 30 days.
