On February 18, 2025 Licensing Program Analyst (LPA) Portia Bowden conducted an unannounced Annual Inspection at the above facility. LPA met with Director Yolanda Carter, explained the reason for visit, provided LIC 125 (Entrance Checklist) and was guided on a tour. Per Director the Facility operates Monday – Friday 6AM-6PM, and does not provide transportation. Also housed on premises is an Preschool Program under facility number 197404640. LPA observed all required documents on the facility licensing boards (PUB 269, PUB 393, LIC 610, LIC 9224, LIC 613A). LPA observed an ER drill log with last Fire drill conducted 4/24.

This is an infant program with a toddler component servicing ages 6 months-2.5 years old, consisting of 1 classroom, 1 infant sleeping room and an outdoor play area. Facility also houses on the premises a preschool program under facility number 197416987. At 2PM, LPA observed 8 infants in care supervised by 2 staff. Per Director there are 12 currently enrolled. Capacity and limitations as specified on the license are being maintained..

The following was observed during the tour of the facility: furniture and equipment were inspected for age appropriateness and good repair. Landline Telephone service is maintained by the facility. LPA observed Central air throughout facility with proper ventilation. At 2:00PM LPA observed an operable Carbon monoxide detector as well as a smoke detector in the preschool nap area. LPA observed sufficient amount of cots for sleeping in the classroom. LPA observed 5 Infant cribs located in the infant sleep room. All cribs were in good working condition with no attachments. Per Director Linens are washed by the facility once a week or as needed. LPA observed 1 Age appropriate sink and 1 age appropriate toilet in good repair in the classroom. Per Director Potty training is provided to LPA observed 1 changing table in good repair within arms reach of the sink in the infant classroom. Per Director parents supply all diapers and wipes.

Per PIN 21-21-CCP lead testing to be conducted for all equipment used for drinking consumption. Per record review, LPA observed negative lead test results conduct on 2/24

LPA observed Disinfectants (pinesol and water solution concocted as needed by facility) stored on a high shelf inaccessible to children in care. Per Director there are no poisons on the premises. Per Director parents supply formula or breast milk until child reaches 12 months at which time facility provides meals provides Breakfast, Lunch and a PM Snack. Per Director there are no children in care with food allergies. Per Director Outside food is permitted LPA reminded Director that all outside food should be labeled with child’s name and dated..

All food preparation, and storage areas are clean, free of litter, rubbish, and rodents/vermin. All food is protected from contamination. All storage containers for solid waste, including moveable bins, have tight fitting covers on and are in good repair.



Outdoor play area is shared with Preschool on a staggered schedule. LPA observed Playground equipment to be in safe condition, free of sharp, loose or pointed parts. The surface of the outdoor activity space is maintained in a safe condition and is free of hazards. There is adequate shade in the play yard. LPA advised that no children shall be left without the supervision of a teacher at any time. All floors were observed to be clean and safe. All materials accessible to children were observed to be toxic free. There are no firearms stored on the premises. There are no pools or bodies of water at the facility.

LPA Reviewed 2 staff files, and observed them to be complete. LPA observed Director Yolanda Carter’s Pediatric CPR/ First Aid with an expiration date of 5/25. All staff had Current Mandated Reporter Training.

LPA reviewed 3 children’s files and observed 3 out of 3 to be missing a sleep log, Per Director she was unaware of sleep log regulations. A type B deficiency will be issued. LPA observed 1 child 10 months old to be missing an Infant Needs and Services Plan, per Director she was unaware of such plan. A type B deficiency will be issued.

The name of the child care center director or fully-qualified teacher(s) designated to act in the director’s absence has been reported to the Department. The person who signs the child in/out of the facility shall use their full legal signature and record the time of day.

Licensee was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for


drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test.
For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP).
LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP.

Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. Facility representative was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption,

Exit interview conducted and report was reviewed with Owner Debra Hawkins. Appeal Rights and a Notice of Site visit was given and must remain posted for 30 days.
