On 07/08/2026 Licensing Program Analyst (LPA) Tyra Chavies, conducted an unannounced  3 Year Annual Required inspection at KinderCare Learning Center. LPA met with director, Derek Wiemnann and toured the facility. Days and hours of operation are Monday through Friday 7:00 AM to 6:00 PM. LPA Chavies confirmed the phone number is: 310-320-4429 and email address is greenwoodave@kindercare.org. There were 32 children in care being supervised by 4 Staff members. This facility license does have a toddler component. If updates to the facility a LIC 200A must be sent in prior to the changes being made. LPA reviewed sign in/ out sheet and all 32 children were signed in at the time of inspection.

This facility has three classrooms, Toddlers, Preschool, and Pre-Kindergarten and all classrooms are currently being used. Parents are to walk their child to the appropriate classroom and ensure teacher is aware the child has arrived. LPA inspected the inside of all three classrooms. Indoor furniture and equipment are in good condition, free of sharp, loose and/or pointed parts. LPA observed cots for sleeping. Sheets will be washed at the center every week and blankets will be taken home every Friday for parents to wash and bring back to the classroom. LPA also observed age appropriate toys and books in primary care area and children's bathrooms. The facility floors, bathroom and classrooms meet Title 22 Health and Safety regulations. If a child is not feeling well, the child will be seated in the front office with a staff member until a parent arrives.

FACILITY EVALUATION REPORT California law requires a public report of each licensing visit/inspection. This report is a record for the facility and the licensing agency. This report is available for public review; therefore, care is taken not to disclose personal or confidential information. Inquiries concerning the location, maintenance, and contents of these reports may be directed to the Licensing Program Analyst or Regional Office whose address and telephone number are listed on the front of this form.

DEFICIENCIES A deficiency is an instance of noncompliance with licensing requirements, including applicable statutes, regulations, interim licensing standards, operating standards, and written directives. Applicants/ licensees must be notified in writing of all licensing deficiencies. Deficiencies are listed on the left side of this form, and the applicable licensing requirement upon which the deficiency is identified. There are two types of deficiencies:
  • Type A deficiencies are violations of licensing requirements that, if not corrected, have a direct and immediate risk to the health, safety, or personal rights of persons in care.
  • Type B deficiencies are violations of licensing requirements that, without correction, could become a risk to the health, safety, or personal rights of persons in care, a recordkeeping violation that could impact the care of said persons and/or protection of their resources, or a violation that could impact those services required to meet the needs of persons in care.

PLANS OF CORRECTION (POCs) The licensing agency is required to establish a reasonable length of time to correct a deficiency. In order to set the time, the licensing agency must take into consideration the seriousness of the violation, the number of persons in care involved, and the availability of equipment and personnel necessary to correct the violation. Applicants/licensees are requested to provide a specific plan for each violation on the right side of the form across from each deficiency. The more specific the plan, the less chance exists for any misunderstanding in setting time limits and reviewing corrections. The applicant/licensee who encounters problems beyond their control in completing the corrections within the specified time frame may request and may be granted an extension of the correction due date by the licensing agency.

CORRECTION NOTIFICATION The applicant/licensee is responsible for completing all corrections and promptly notifying the licensing agency of corrections. Applicants/licensees are advised to keep a dated copy of any correspondence sent to the licensing agency concerning corrections, or if corrections are telephoned to the licensing agency, the date, person contacted, and information given.

CIVIL PENALTIES The licensing agency is required by law to issue a Penalty Notice, when applicable, to all facilities holding a license issued by the licensing agency, or subject to licensure, except Certified Family Homes, Resource Families, and Foster Family Homes, or any governmental entity.

PENALTY NOTICE GIVEN The statement concerning civil penalties serves as a penalty notice on this Licensing Report and failure to correct cited licensing deficiencies will result in civil penalties. Applicants/ licensees are required to pay civil penalties when administrative appeals have been exhausted and in accordance with any payment arrangements made with the licensing agency.

APPEAL RIGHTS The applicant/licensee has a right without prejudice to discuss any disagreement in this report with the licensing agency concerning the proper application of licensing requirements. The applicant/ licensee may request a formal review by the licensing agency to amend or dismiss the notice of deficiency and/ or civil penalty. Requests for review shall be made in writing within 15 business days of receipt of a deficiency notification or civil penalty assessment. Licensing deficiencies may be appealed pursuant to the procedures in the LIC 9058 Applicant/Licensee Rights.

AGENCY REVIEW The licensing agency review of an appeal may be conducted based upon information provided in writing by the applicant/licensee. The applicant/licensee may request an office meeting to provide additional information. The applicant/licensee will be notified in writing of the results of the agency review within 60 business days of the date when all necessary information has been provided to the licensing agency.

EMAIL REQUIREMENT Adult Community Care Facilities, Residential Care Facilities for the Chronically Ill, and Residential Care Facilities for the Elderly are required to provide and maintain an active email address of record with the licensing agency.

There are three different playgrounds. The playgrounds are located in the back of each classroom and all playgrounds are currently being used. LPA toured the outdoor activity space. The surface of the outdoor activity space is maintained in a safe condition. Areas around climbing equipment have cushioning material to absorb falls. All materials and surfaces accessible to children are toxic free. Facility has one or more functioning carbon monoxide detectors/ smoke alarms. The facility does have one or more fire extinguishers which has an inspection date of January 20,2026 on the premises. There are no bodies of water on the premises. There are no firearms or ammunition allowed or stored on the premises. Disinfectants, cleaning solutions, medication and other hazardous items are made inaccessible. All poisons are kept in a locked storage area.

During an inspection dated 09/17/2025, LPA Chavies received the following documents from facility: lead test results from January 2023 (which verifies sample testing over 5.5 ppb/ug L) LIC 9275, LIC 9276, Facility Sketch (water faucets/outlet identified), lead test results from May 2023 (which verifies sample testing under 5.5 ppb/ug L), LIC 9275, LIC 9276, Facility Sketch (water faucets/outlet identified). This facility is currently in compliance.

The Kitchen was inspected and is maintained in good condition. AM/PM snacks are provided by the facility. Drinking water is available both indoors, outdoors and upon request. LPA observed children snacking. All food was safe, of the quality and in the quantity necessary to meet All food is selected, stored, prepared and served in a safe and healthful manner.

POSTING REQUIREMENTS: License and other relevant notices are visible for public view and correctly posted on the wall (Facility Sketch, PUB 394, LIC 9148, LIC 610A). Fire and disaster drills are being conducted as scheduled every month.

CHILDREN'S RECORDS: LPA Chavies reviewed 10 children's files. All files currently have all required Forms (LIC 700, 701, 995E, 627) and immunization records. LPA advised director to have a written plan for conducting Daily Health Checks.

STAFF'S RECORDS: LPA Chavies reviewed 5 staff files. All staff files have all required LIC Forms (LIC 503, 508, 9108, and 9052). One or more persons currently have a Valid Pediatric First Aid and CPR Certificate and all Mandated Reporter Trainings are valid.

Director was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.


Commencing September 1st, 2016, prohibits a person from being employed or volunteering at a childcare facility or family day care if he or she has not been immunized against influenza, pertussis and measles.

Law enacted by SB 277, beginning January 1st, 2016, personal beliefs exemptions will no longer be an option for the vaccines that are currently required for entry into childcare or school in California. Personal beliefs exemptions already on file will remain valid until the child reaches the next immunization checkpoint.
A licensee may file an appeal, in writing 15 business days from the date of receiving the penalty assessment.
When regulations are approved/changed/updated, providers will be notified through Provider Information Notice (PIN).
