13 | On 07/06/23 Licensing Program Analysts (LPA) Justin Dorsey conducted an complaint investigation at the facility to deliver complaint investigation findings. Upon arrival LPA met with Sister Catalina Avila. LPA Dorsey observed 0 children in care.
During the course of the investigation LPA Dorsey interviewed thecomplainant, children, and staff. As part of the investigation LPA Dorsey obtained the facilities children roster and documents relevant to the investigation. After observations and interviews with parties related to the allegations it was found that the allegations could not be collaborated. Therefore, the allegations have been found unsubstantiated. Although the allegation may have happened or is valid, there is not a preponderance of the evidence to prove that the facility is not providing a safe and healthful environment, Therefore the above allegations are Unsubstantiated.
An exit interview was conducted, and a copy of this report was provided to Sister Catalina Avila along with Notice of Site Visit and Appeal Rights.
