On 6/15/2022 8:50AM, Licensing Program Analyst (LPA) Denise Miranda, conducted an unannounced Required 1- Year Inspection to the facility. At 8:57 AM, LPA met with Director Assistant Ms. Christine Chiche. According to the Director Assistant, the facility operates: Monday – Friday from 7:00am – 6:00pm. At 9:05 AM. LPA were guided on a tour of the facility inside and outside. Facility checklist (LIC 125) provided upon arrival to Director.

There is a private elementary school on the premises- K through 5 grades. Facility is licensed for 150 preschoolers ages – 2-6 years old. LPA Miranda observed 71 preschooler children under supervision of 16 staff and Director Assistant. LPA observed all children are under supervision, including visual supervision, of a teachers at all times.

There are two a swimming pools on the premises, per Director Assistant only the swimming pool name on sketch as (wading pool), is on limit to the preschool children in care. Children were using the yard K &1 grade yard only for access the swimming pool area (wading pool). There is fenced per regulation. The pool gate is self-latching, self-closing and opens away from the swimming pool.

There are no firearms or ammunition allowed or stored on the premises. Disinfectants, cleaning solutions, medication and other hazardous items are made inaccessible. All poisons are kept in a locked storage area. No poisons were observed during the inspection.

Furniture and equipment are in good condition, free of sharp, loose or pointed parts. Playground equipment is in safe condition, free of sharp, loose or pointed parts. The

(1) On or before March 30, 2018, a person who, on January 1, 2018, is a licensed child day care provider, administrator, or employee of a licensed child day care facility shall complete the mandated reporter training provided pursuant to paragraphs (2) and (3) of subdivision (a), and shall complete renewal mandated reporter training every two years following the date on which he or she completed the initial mandated reporter training.

Based on record review and interview with Director Assistant, the licensee did not comply with the section cited above in 4 out of 17 staff files reviewed show staff are in need of mandated reporter training which poses/posed a potential health, safety or personal rights risk to persons in care.

surface of the outdoor activity space is maintained in a safe condition and is free of hazards. All toilets and hand washing facilities are in safe and sanitary operating condition. Floors in the facility are clean and safe. All kitchen, food preparation and storage areas are clean, free of litter/rubbish and free of rodents/vermin. All food is protected against contamination and any contaminated food is discarded immediately. Solid waste storage containers have tight-fitting covers and are in good repair. Drinking water is available both indoors and outdoors. Areas around high climbing equipment, swings and slides have cushioning material to absorb falls. The facility is free of flies, insects and rodents. Facility has one or more functioning carbon monoxide detectors that meet statutory requirements. Prior to working or volunteering in a licensed child care facility, all individuals subject to a criminal record review have received a criminal record clearance or exemption.

LPA observed that facility has a space for animals such as chicken, pig, donkey, duck, rabbit, the animal have their own space and all area has been barricade. Per Director Assistant, facility has lizards and snakes, that facility keeps inside of a shed and boxes. Per Director Assistant, children access the ranch/ the animal areas with supervision and do not access the animal’s cage.

Capacity and limitations as specified on the License are being maintained.

At least one person trained in CPR and Pediatric First Aid is present when children are at the facility or at offsite activities. The name of the child care center director or fully-qualified teacher(s) designated to act in the director’s absence has been reported to the Department. The person who signs the child in/out of the facility shall use their full legal signature and record the time of day.

All children are under supervision, including visual supervision, of a teacher at all times. Facility maintains a ratio of one teacher supervising no more than 12 children in care. LPA reviewed a sample of 10 children’s files and observed files were complete with contact information for authorized representative and or relatives or others who can assume responsibility for the child and medical assessment. LPA reviewed all staff files and observed files were complete with health screening, immunization records for influenza,

Criminal Record Statement

Director was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

Incidental Medical Services (IMS) policy was discussed.


Regulation and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. LPA and Licensee discussed the Community Care Licensing website www.ccld.ca.gov which will provide access to Provider Information Notices (PINs), Quarterly Updates, COVID-19 Information and Resources, Mandated Reporter Training, Safe Sleep in Child Care, Lead Poisoning Facts, Forms and Regulations.
LPA, Miranda conducted 2:50PM staff interviews during today's visit.

Per Title 22, Division 12, Chapter 3, of the California Code of Regulations, the following deficiency is being cited: (see next page, 809 D) Licensee was provided a copy of their appeal rights.

A notice of site visit was given and must remain posted for 30 days.

Exit interview conducted and report was reviewed with the Director Assistant, Christine Chiche.
