Per Staff#1 (S1) interview, there are always two staff present providing care to the children. Per staff interview, the only time a staff member stays alone with the class is when children are absent, and they have a low ratio, or are in ratio. Based of LPAs visit, LPA observed two teachers present at all times in the class, the director and the teacher. LPA did not observed the facility to operate out of ratio. Per children interviews, they are familiar their teachers and state they have 2-3 teachers. Per S2, sometimes S3 helps with support, but the facility is typically low in ratio. Based of parent interviews, they observe 2-3 staff members present at the facility.
Per S1 and S2, they use communication are a form of discipline. When interviewing S1 and S2, they disclosed they sit down with the children for about 3 minutes and talk to them about their behavior. Per S1 and S2, they talk to children about their actions and feelings. Per S2, they also inform children's parents about the child's behavior. Per S2, Parents are called via phone call and receive a written report on the child's behavior. Per S2, parents receive children's behavior reports at the end of the day. Based on Children interviews, C1, C3, and C6 , they disclosed that when they are misbehaving or other children in the class misbehave they have to take a break for a few minutes. During LPAs visits, LPA observed no inappropriate form of discipline.
Based on S1 and S2, children only use the preschool licensed areas. Per S1 and S2, children use the classroom, the playground, and the music room. Per S2, they walk through the school hallway to go from one place to another. Per S1, children go to the filed during fire drills but are never in off limit areas. During LPAs visit, LPA observed children to be in the preschool class or in the preschool playground. When interviewing children, they were familiar with the playground they play in.
Although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation(s) did or did not occur, therefor the allegation is unsubstantiated,
The Notice of Site Visit (LIC 9213) – must remain posted for 30 days during the hours of operation after each site visit by a licensing representative. Failure to maintain posting as required will result in a civil penalty of $100.00. Exit interview was conducted with the director, Rosemary Elias, including, but not limited to Provider Rights, Appeal Procedures and Agencies Consultative Role. |
