On 6/4/2024, Licensing Program Analyst (LPA)Tyra Chavies and Lisa Clayton, met with Pauline McPeake and Ceceilia Carrillo conducted an unannounced case management inspection to follow up on the Director qualifications request submitted to the Department for review. During the visit LPA observed 11 infants being supervised by 5 staff.

During the visit LPAs received transcripts and course descriptions documents for further review.

Ms.Carrillo and Erika Byrd (Office Manager) will provide revised LIC 509, LIC 9108, LIC 9096 and documentation supporting site supervisor position no later than Friday June 14,2024.

An exit interview was conducted with Ms. Carrillo and Ms. McPeake.


A copy of this report was read and given to Director McPeake.
A notice of site visit was provided and must be posted for 30 days.
