25 | On 03/07/2025, Licensing Program Analyst (LPA) Justeene Tamayo met with Administrator Melanie Culver and School Readiness Coordinator Joe Vega-Smith, who granted access to the facility. The purpose of the inspection was to conduct a unannounced case management inspection for a UIR received at the Palmdale Regional Office on 11/13/24. There were 27 preschool children, and 6 teachers during today's visit.
Description of Incident: On 11/13/24, classroom room A was flooded by the children's toilets. Facility needed to repair the entire classroom due to the flooding. LPA Tamayo was informed the repairs of classroom room A has been completed effective 03/03/25.
During this follow-up inspection, LPA Tamayo interviewed the Administrator and School Readiness Coordinator to gather additional information. Due to the severity of the issues in Classroom A, extensive repairs were required, including the complete replacement of the subfloor and walls. It was revealed that a remediation company, Citadel Environmental Service Inc., had previously inspected the classroom and conducted mold testing. Initial tests showed the presence of mold, prompting Burns Environmental Services to conduct a second round of mold testing to ensure the area was clear of mold. On 01/24/25, Burns Environmental Services confirmed that no mold remained in Classroom A. A finalized mold report will be emailed to LPA Tamayo no later than 03/14/25.
Additionally, from 01/27/25 to 02/10/25, repairs and renovations took place in Classroom A after the clearance of mold. During this visit, LPA Tamayo inspected both the classroom and the bathroom, finding them to be in compliance.
