On January 15, 2025, Licensing Program Analyst (LPA) Annelise Villa conducted a follow up complaint investigation related to the allegations above. LPA disclosed the purpose of the investigation and was granted entry into the facility by Director Amy Curtis. A tour of the facility was conducted. LPA verified a census of 6 infants and 4 staff working under the facility license.

The allegation states the facility has unqualified staff providing care and supervision to infants in care. During the investigation, LPA conducted interviews and record review and found the following was revealed: Director stated the personnel files are before hiring infant teacher staff; however, the facility did not request official transcripts from the infant teacher hires. Record review showed that Staff #5 is an unqualified infant teacher who provided care and supervision as a fully qualified infant teacher at the facility without having completed any ECE semester/quarter units. Based on information obtained, observations, and interviews with relevant complaint parties, the allegation is deemed substantiated. A Type B citation is being issued. A finding of substantiated means that allegations were valid because the preponderance of the evidence standard has been met. As a result of the investigation, the facility was found to be in noncompliance with Title 22 Regulations and has been cited deficiencies, see the LIC 9099-D for the details.
