On 02/24/25 Licensing Program Analyst (LPA) Portia Bowden conducted an unannounced Annual Inspection at the above facility. At 9:00AM LPA met with Licensee Carmelita Thomas, provided her with an entrance check list and conducted a tour of the home. Also present was Co Licensee Darren. LPA observed 4 children in care during inspection. Per Licensee there are 14 children currently enrolled. Per Licensee hours of operation are 6am-6pm Monday- Friday. Per licensee she is apart of the food program and provides Breakfast, AM Snack, lunch, and PM Snack. Per Licensee outside food is permitted in the facility. LPA reminded Licensee to ensure all outside food is labeled with child’s name and date. Per licensee, overnight care is not provided while transportation from school is provided. LPA observed all appropriate documents on licensing board (Facility License (LIC 203), Emergency Disaster Plan (LIC610a), ER drill log last dated 5/24, Notification of Parents' Rights Poster (PUB 394), Child Care Facility Roster (LIC9040). LPA observed both Licensee’s expired Pediatric CPR/First Aid Certification with an expiration date of 5/24. LPA observed both Licensee’s expired Mandated Reporter Certificate dated 6/24, per licensee both licensee’s will obtain current CPR and Mandated Reporter certificates and submit to LPA, a type B deficiency and a technical violation will be issued.

Individuals residing in the home have been discussed and noted. All adults present in the home have obtained a criminal record clearance or exemption. All areas identified on the facility sketch that are accessible for children to use were inspected for safety, comfort, and cleanliness. The following was observed and reviewed during this inspection:

This is a Two-story home consisting of four bedrooms, 2 bathrooms, living room, kitchen, front Yard, enclosed backyard and attached converted garage. Per Licensee areas used by the children include the kitchen, living room, one bathroom located in hallway next to garage, attached garage for activities and the enclosed backyard. Per Licensee areas off limits to children and parents include all 4 bedrooms and 1 bathroom upstairs. LPA observed a baby gate preventing access to stairs.

Per Licensee there are no poisons on the premises. Per Licensee Detergents, are kept in the laundry area in the garage inaccessible to children in care. Per licensee medication is stored in an off limit bedroom upstairs, inaccessible to children in care. At 9:30AM LPA observed a fully charged 2A10BC fire extinguisher on the wall of the garage with a service tag dated 3/24. At 9:35AM LPA observed a functioning smoke detector and a functioning carbon monoxide detector on the ceiling of the living room. The home was observed to have proper ventilation and heating for safety and comfort. LPA observed a fireplace in the living room properly barricaded. The home maintains telephone service via cell phone. Per licensee there are no firearms on the premises.

Kitchen area was observed to be clean and free of clutter and have no sharp objects or chemical compounds accessible to children in care. At 9:40AM LPA observed kitchen knives stored in a high kitchen cabinet, inaccessible to children in care. Bathroom children was observed to be clean and free of hazards. LPA observed the cabinet below the bathroom sink to be stocked with hand washing soap and wipes. Per Licensee she provides potty training.

LPA observed adequate toys and age appropriate learning materials in the garage as well as mats for sleeping. Per Licensee all sheets and blankets are washed by the facility twice a week and as needed.

Outdoor Play for children in care is located in the enclosed backyard. At 9:50AM LPA observed the area to be fully enclosed with age appropriate outdoor toys, and proper shading. No bodies of water were observed on the premises.

4 children’s files were reviewed and observed to be complete.

Licensee was reminded that all adults 18 and over living or working in the home, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

LPA discussed the safe sleep regulations with licensee and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02-CCP. When any IMS is provided, a Plan for Providing IMS must be submitted to the Department. Licensee was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

During the exit interview, Licensee Carmelita Thomas, confirmed there are no Registered Sex Offenders living in the home. LPA completed the RSO profile in FAS.

1 Type B deficiency was observed during today’s inspection for expired CPR certificates was issued.
